Last week, the FDA announced a ‘boxed warning’ & new discussion points with patients regarding side effects.

It is NOT an antihistamine.
It has NEVER been a 1st line treatment for asthma/allergies.
It is commonly misused by itself to treat allergic rhinitis, which is not effective. New FDA warning encourages not using as such.
The new FDA warning comes after longer term evidence of these previously known rare side effects.
This isn’t ‘new’, but is more proactive.
-These are rare
-Typically occur in 1st 1-2 months of use
-Watch for sudden change in behavior, nightmares, depression
-Typically reversible once stopped
-If currently using w/o problems, likely ok
-Montelukast should be used thoughtfully in patients w poorly controlled asthma or allergic rhinitis as ADD ON to other proven meds
-Don’t use by itself for mild allergies
-All patients should receive info re: possible side effects

This new boxed warning will hopefully help prescribers be more proactive in discussions with patients...and more thoughtful about when to consider montelukast
