PATELOFEMORAL SYNDROME: a person with FM, EDS, POTS, IBS has underlying Hypermobility Spectrum Disorder, and likely had Genu valgus or varum. This potentiates Patellofemoral Femoral Syndrome (PFS). See my threadreader library for a thread about PFS.

“I have fibro, EDS , POTS,IBS, pernicious anemia. Currently struggling with knee pain to side of kneecap on inside of right leg. Any tips on how to help myself during lockdown. I've been trying heat its from overdoing it 3 weeks ago. Any exercises to try?”
Patelofemoral Syndrome is an asymmetric posture of the knees whereby either the medial compartment or lateral compartment becomes more burdened by upper body weight, & associated collateral ligaments become similarly bio-mechanically stressed & pained.
Solution would be to change 3-D configuration of knee to reduce Genu morphology; with a bolster (shoe insert) to jack up either lateral or medial plantar surface of foot. This would tend to shift knee joint to a more central station.
People with the valgus knee deformity will compensate by walking on outside of foot (supinate the foot) in case of valgus in attempt to normalize knee morphology & posture. Examination of wear pattern of sole of shoe will confirm.
Persons with valgus morphology would experience medial compartment compression & lateral collateral ligament stress. Medial knee pain might result. Standing knee X-rays might show the medial compartment compression.
Another way to confirm the musculoskeletal bio-stress is to go to The Walking Co. Shoe store in the mall and take their walk test over sensors & see the pressure loci of foot in color assisted images.
In cases of knee valgus, a shoe insert that elevates the medial foot, the arch, will tend to reduce pressure on the medial knee compartment & lateral collateral knee ligament. Over time the pain is expected to remit.
In cases of varum, a similar bio-mechanical analysis can be undertaken to arrive at a properly stationed shoe insert.
A knee supporter with a window for the patella can be obtained at CVS store orthotic sections.
What I tell people who have loose ligaments and PATELOFEMORAL SYNDROME is that since they have loose ligaments supporting their joints, they need to develop gorilla muscles around the joint; as compensation.
Muscle development of specific muscle groups is a skill set held by Physiotherapists, and visit with one of these clinicians is advised.
