In short, it is not transnational, it has no border operations, it has 10,000 members (same as it did a decade ago--which is miniscule for a gang in the US), and most all of its victims are immigrants or children of immigrants (Trump lied about them killing a police officer).
There are real horrors here, and grotesque violence. The question is why white Republicans suddenly care now. Well, because they've been told to, and because a violent Latino gang is a useful cudgel for talking "tough" about the border.
And, of course, to create the utterly false impression that MS-13 is "streaming over the border" when their transnational presence is nil. The scaremongering campaign is also meant to create a false impression that MS-13 will kill white citizens. It's racism all the way down.
The gang's actual victims include many undocumented people. The very people who are being made to live in terror of the authorities, and who are being ruthlessly targeted by Trump and his supporters as well. Therein lies one of the greater tragedies here.
