Just finished inquest number 2 of this month. Jury concluded there were three serious failings which contributed to a suicide at HMP Birmingham before it was taken over from G4S by Ministry of Justice. Will tweet report when it’s up.

The jury concluded (1) he should not have been given the pen ‘in possession’ (faulty risk assessment by doctor), (2) he should not have been discharged from mental health and (3) the ACCT (suicide prevention) systems broke down. I acted for John’s family with @TuckersLaw 
This took place on early 2018, a period where G4S were doing such a terrible job of running HMP Birmingham that it was taken over in August by the government. A number of deaths took place in a short period, including that of my client’s brother bbc.com/news/uk-englan…
A number of staff told the coroner they were under pressure due to staff shortages. Some didn’t know basic procedures.
Should add that this was an ‘article 2’ inquest - expanded investigation made possible because of the Human Rights Act which requires deaths in state custody to be investigated in detail to prevent them happening again. Before the HRA this would have been a cursory investigation
