Noma medical expert Denise Baratti-Mayer says that noma disease progression is very quick. Unfortunately, most children and families seek care too late, once lesions are no longer reversible.
There are still unknowns about the etiology of noma. Noma appears to be a non-contagious opportunistic infection, probably caused by commensal bacteria, and strongly associated to other diseases or poverty (i.e. malnutrition, past measles infection, poor oral hygiene).
Denise Baratti-Mayer concludes her presentation with epidemiological numbers. Incidence of noma is estimated to be between 30,000-140,000 per year, with a very high mortality rate.
There are up to 700,000 noma survivors around the world.
Short and strong message by @drEOEhanire, Minister of Health of Nigeria 🇳🇬. This country is very seriously addressing noma and leading global advocacy efforts.
Noma expert Klaas Marck reminds us that there were many cases of noma in Europe until the XIXth century. Policies promoting WASH and hygiene, as well as nutritional support, were critical to eradicate noma in Europe.
This is a map of reported cases of noma in recent history. But it is actually not completely accurate. You'll see that Lao 🇱🇦 is not coloured, although Leila Srour has identified many noma cases in rural Lao over the last two decades.
Burkina Faso 🇧🇫 is another country now committed to tackling noma. Head of noma control program Rose Drabo describes the response at national level.
20 years ago, adventurer @bertrandpiccard saw a young girl with noma for the first time. He initially thought that her face had gotten hit by an axe!
Since then, he's been a tireless advocate for recognition of noma as a public health priority.
Noma survivor Mulikat Okanlawon from Nigeria 🇳🇬 tells her story. She came all the way from Lagos to receive surgery in Sokoto. In spite of stgmatization, she says that "all of us can raise awareness of noma disease. After surgery, there is life".
.@IoanaCismas presents her project on noma: better estimate burden of disease, better evaluate the social impact of noma. Noma patients and survivors very often suffer from discriminations and rejection.

Bukola Oluyide tells the stories of several noma patients treated in Sokoto and describes the holistic approach taken by #MSF: surgery, nutrition, mental health support, physiotherapy, awareness, operational research.
Surgery and facial reconstruction are critical interventions for young children with irreversible lesions caused by noma. But these interventions present many technical challenges in resource-limited settings, says Issa Hamady from @HilfsaktionNoma.

Last speaker is @makinooon3. Actions by @WHOAFRO against noma are part of the oral health program. Progress has been made, but it would be great to see more sectors involved, not just oral health.

These new results confirm similar positive results for calcifediol generated in Cordoba in a smaller pilot RCT

The new study in Barcelona has limitations: no placebo (open label); randomization per ward (not individual); modified intention to treat for some patients.
Yet results are very encouraging !
It was apparently submitted to @TheLancet. I hope it will get published soon.

Bangladesh: large RCT (n=606) of #mouthwash, gargling, nasal spraying & eye drops w/ 1% povidone-iodine (PVP-I, aka #Betadine) for #COVID_19. Every 4 hours for 4 weeks.
Impressive results.

Mouthwash solutions of PVP-I can be easily purchased OTC in many countries. Nasal sprays and eye drops are less easily accessible (i.e. not available OTC in France).
There have been other in vivo and in vitro studies over the last months. A tiny RCT from Singapore had previously found encouraging results.

Ivermectin belongs to the family of avermectins, which are natural fermentation products of soil bacteria. A Nobel Prize of Medicine was awarded for their discovery. Image

Ivermectin is indicated for several parasitic infections: onchocerciasis, lymphatic filariasis, strongyloidiasis, scabies. It works on head lice too. #NTDs

But ivermectin may have a broader therapeutic benefit. Hence its nickname, the “wonder drug”.

In this thread, I’ll speak about #tocilizumab and challenges to access it. Tocilizumab is an anti-IL-6-receptor monoclonal antibody. It is a promising investigational drug to treat the cytokine storm associated with severe #COVID19.

The cytokine storm (aka cytokine release syndrome) occurs when a high number of white blood cells are activated and release inflammatory cytokines, such as IL-6, which in turn activate more white blood cells.

Tocilizumab is not an antiviral agent. Like other host focused therapies for COVID19, tocilizumab can be used in association with an effective antiviral and their combined actions may be complementary.

Remdesivir (aka GS-5734) inhibits RNA-dependent RNA polymerase. Broad-spectrum antiviral. Injectable (IV). Wasn’t found as clinically effective for #Ebola as monoclonal antibodies. Here is the formula. The synthesis of GS-5734 is described here. pubs.acs.org/doi/full/10.10… 2/n
How much of remdesivir is available now ? Unclear. In Feb 2019, when drug was still a promising candidate for #Ebola, Gilead planned to have 25kg + 4x40g of (unfinished) drug substance by 2020, equivalent to 148,000 treatment courses. Unknown if it materialized. 3/n
