1/🧵 #WISDOM of Wine, Coffee & Chocolate. 🍷 🍫 ☕️

Millions love morning coffee or curling up w DARK chocolate & wine

What’s the #SCIENCE?

All 3 in moderation are safe…

This thread will focus on WINE! Let’s explore actual SCIENCE…
2/ This infographic shows the putative benefits – weight & BP control, diabetes & stroke prevention. But what do actual data say about this?

3/ Wine has antioxidants and flavonoids that might reduce cardiac disease.
bit.ly/3LtmGEG

Resveratrol from grapes inhibits initiation, promotion, and progression of cancer.

4/ A massive study in ~25,000 patients & >257,000 person-years (Copenhagen Heart) tracked beer, wine & spirits; smoking, physical activity, education, BMI.

All-cause mortality, cardiac, & cancer related deaths ⬇️ w moderate use of 1 to 2 drinks/day.

5/ The famous Framingham Heart Study also showed a reduction in heart failure with moderate drinking of wine up to ~1.5 drinks/day.

bit.ly/3BjGfuM
6/ 38,000 male health care professionals free of heart dz were studied over 12 yrs & 1,418 heart attacks. The type of beverage didn’t matter – beer, white, red, or liquor. A 12.5g ⬆️ in daily alcohol consumption reduced heart attacks by 22% over 4 yrs.

7/ In an Extension of the Physicians Health Study of 89,000 men (N=14,125 with a  Hx of HTN), it was shown that as compared to NON-drinkers, monthly, weekly & daily alcohol use cut risk of cardiovascular death by up to 50%.

8/ A study of the psychological effects of drinking showed that beer drinkers had lower IQs, more neuroticism and psychoticism, higher levels of lying, and more overt abuse.

bit.ly/3BiCaab
9/ The same study showed that WINE drinkers had higher parental education, socioeconomic status, less alcohol abuse, and optimal functioning on personality tests, psychiatric symptoms, and health-related behavior.

bit.ly/3BiCaab
10/ The #nurse health study of N=12,480 nurses showed that 1 drink per day of beer or wine had 25% ⬇️ cognitive decline (note risk is lower - on LEFT👇) and there was no increase even up to 2 drinks per day. This is very encouraging for brain health!

bit.ly/3uLaQzO
11/ Light to moderate 🍺 🍷
(1.5 drinks/day) reduced risk of a Stroke by 25%. But increased drinking up to 3 to 4 drinks per day made risk go UP by 75%.

J-WAVE: it’s another example of the J wave of concern. Moderation is good. Excess is bad.

12/ Should diabetics drink alcohol in moderation?

A 2-year study of 224 stable Diabetics randomized to 150 ml of water, white wine, or red wine. All on Mediterranean diet.

Wine was superior to water in reducing lipids & controlling sugars in diabetics.

13/ CONCLUSIONS:
Wine or combinations of alcohol in these studies were STRONGLY associated w reduced morbidity & mortality from all-causes, heart, cancer, and stroke risk.

The J-shaped curve has been consistently proven.

That means =
Everything in moderation = Happier life!

1/🧵🎥 Politicians & Media are sending messages to take down #COVID protections.

In US it’s premature. Why?

📍>50% adults are NOT protected w #Booster
📍>75% of kids 5-11 are NOT fully #Vaccinated
📍Feb 8th >3,000 died
📍>17,000 dead in past week
📍>60,000 💀 January

Thoughts?
2/ Last week I had moments where I thought, “It really is sad that society has to keep masking up & those poor kids in school haven’t seen people smile in so long.”

THEN…I thought about how dangerous it is for #CovidKids who get #longCovid to have to live that way.

And…
3/ People continue to contract #Covid from children who skate through #Omicron initially while adults spend 3 weeks in ICU & die.

Denmark 🇩🇰 Israel 🇮🇱 South Africa 🇿🇦 will be safer faster.

US 🇺🇸 will lag behind by YEARS.

Messaging about an end to the pandemic is premature!

1/🧵ICU Burn-Out Prevention in #COVID: a few thoughts

Early on as an ICU Doc, I carried a lot of shame because I realized that even though I sufficed as a technician and possessed the right knowledge, I was NOT doing a good job as a “person” with “people.”

#burnout #nurse Image
2/ Which is to say, I was only “good” doctor on paper. NOT in real life.

I kept myself at too much of a distance, and I thought my job was to do procedures well and provide the right medicines.

Now I know that was not enough, not by a LONG SHOT.
3/ 📚 I wrote: “That shot of adrenaline from saving a life dissipated once the ventilator was in place…as if we handed over care of our patients to machines while we went off & intubated, sedated & paralyzed the next person...

1/🧵 #COVID Brain Changes Show Parallels w Alzheimer's Disease

Findings may point to potential mechanism for #brainfog in people with #longCOVID

2/ Brains of COVID-19 patients had some of the same pathological changes seen in Alzheimer's disease, which may explain the memory problems people with long COVID experience....

3/ The study...linked the inflammatory response found in SARS-CoV-2 infection with pathways causing tau hyperphosphorylation typically associated with #Alzheimers. The data also indicated a role for leaky ryanodine receptor 2 (RyR2) in the pathophysiology of SARS-CoV-2 infection.

1/🧵 Do you think this is WRONG⁉️

Would you be upset if you found out that a medical student had done a FULL PELVIC EXAM (or PROSTATE EXAM) under general anesthesia without explicitly telling you beforehand or even afterward?

Is this a violation of your privacy?

Read on…
2/ Would you consider it A FORM OF ASSAULT if you found out that a medical student had done a FULL PELVICE EXAM (or PROSTATE EXAM) under general anesthesia without explicitly telling you beforehand or even afterward?
3/ Apparently, some medical schools are still doing this as a way to teach budding doctors how to examine the human body. Many types of injustice in medicine have occurred over the years, and this represents one of them.

1/🧵 What is #LongCOVID⁉️

A disabling malady from ongoing inflammation, autoimmunity, & potential viral reservoirs (GI, brain?)

NEW DATA: The Lungs “light up” on special MRI Scans 3 to 9 months later in patients never hospitalized for COVID.

bit.ly/344Oajp
2/ These #LongCOVID pts had NORMAL CT scans but when asked to inhale Xenon, an inert gas, the MRI showed their lungs transferred gases (think oxygen) into the bloodstream abnormally. This is alarming evidence of another weird form of organ damage from COVID not visible on CXR/CT.
3/ Oxford Prof Dr. Gleeson says these patients, who were never in hospital & didn’t have acute severe illness w their COVID19, have been experiencing symptoms for a year after contracting the infection.

1/🧵 Covid-19 #vaccination in American Indian & Alaska Native (AI/AN): @NEJM

The AI/AN population has the highest 1st dose & full #vaxx rates of any US racial or ethnic group.

This photo of Black Elk should remind us of true #HEROES

link: bit.ly/3KY77og Image
2/ This successful trend is especially important given AI/AN communities have been disproportionately affected by Covid-19 and by ongoing discrimination throughout the health care system.

AI/AN communities have worked to protect the health and well-being of their members by… Image
3/ …adapting community mitigation & vaccination strategies to their diverse cultures.

There are 574 federally recognized tribes located in 35 states. AI/AN lands, which are sovereign territories, extend over 100 million acres, including 44 million acres in Alaska.
