Also: du erzählst was & plötzlich is Schluss & du bist lost.
- Wortfindungsstörungen - hatte ich

1/🧵 Let’s talk Long COVID:

I’m grouping several important & interesting articles about the reality of #LongCOVID.

By studying brain fluid, viral persistence & MRIs, we’re building our understanding of the “how” & “why” of this devastating malady.

A train is coming, read on…
2/ Cerebrospinal Fluid Offers Clues to #BrainFog

Most pts w new “thinking problems” after mild #COVID had abnormalities in fluid from lumbar punctures (image); controls did not.

Delayed #LongCOVID was common in younger people who thought they were safe.

3/ Believing those with #LongCOVID complaints is our 1st basic show of support!

“If people tell us they have new thinking & memory issues, I think we should believe them rather than require that they meet certain severity criteria.”

@LongCovidKids @long_covid

1/🧵 #COVID Brain Changes Show Parallels w Alzheimer's Disease

Findings may point to potential mechanism for #brainfog in people with #longCOVID

2/ Brains of COVID-19 patients had some of the same pathological changes seen in Alzheimer's disease, which may explain the memory problems people with long COVID experience....

bit.ly/3sr1S8p
3/ The study...linked the inflammatory response found in SARS-CoV-2 infection with pathways causing tau hyperphosphorylation typically associated with #Alzheimers. The data also indicated a role for leaky ryanodine receptor 2 (RyR2) in the pathophysiology of SARS-CoV-2 infection.

1/🧵 How do #COVID patients suffer after the ICU?

This new @JAMA_current study of 246 Dutch pts shows that 3 out of 4 people had physical problems 1 YEAR LATER plus 1 in 4 have #brainfog, depression & anxiety.

What do we call this?

#LongCOVID #PICS

bit.ly/3o4djkT
2/ The paper is also discussed in @Medscape👇: “Our study shows what an impact ICU admission has on former #COVID19 pts. Even after 1 year, half are tired or experience lack of the energy to fully resume their work," says author Marieke Zegers, PhD

wb.md/33U9DeF
3/ I searched this paper for well-known terms like post-intensive care syndrome #PICS, #LongCOVID & even “post-acute sequelae of COVID”#PASC

NADA. Zip. Zilch.

Why weren’t these names mentioned? At the very least, these pts’ disabilities are DEFINITELY PICS & underestimated btw

Dear @UWaterloo about this hardline on returning to F2F Feb 28. A major element of #LongCovid seems to be microglial inflammation (that’s in the brain) yielding neurological damage. The prevalence of long COVID varies among studies, but 5-10% is a conservative estimate. 1/3 🧵
A #University’s greatest asset is their people. You’ve invested so much in all your #faculty. Mostly we are valued for our #brains. I wonder if those pushing the “learn to live with COVID” agenda have done the math on what it would cost if 5% of us got #LongCovid #NeuroCOVID 2/3
Even 3+ months of #brainfog in 100 of your highly skilled faculty must be $$$. Add to that our 6200 grad #students and 36000 undergrads here to develop their brains. Do they know the gamble they are taking with their Neuro #health? @yourWUSA I don’t get the rush 🤷🏻‍♀️🧠 #COVID19 3/3

Reihenweise Impfdurchbrüche bei vorsichtigen, geboosterten Personen, med. Personal +Andere.

“In a nutshell, this study illustrates that respiratory-only mild SARS-CoV-2 infection can lead to detrimental changes in the brain, likely mediated by inflammatory factors. Similar neuropathobiology may be shared in chemo-brain, post-ICU syndrome and ME/CFS”
Mechanisms and potential for further research and treatments also discussed
“"The findings... illustrate striking similarities between neuropathophysiology after cancer therapy and after SARS-CoV-2 infection, and elucidate cellular deficits that may contribute to lasting neurological symptoms following even mild SARS-CoV-2 infection"

1/7 Why I care about #MECFS : My husband had the illness since 2012 when he was infected with the Epstein-Barr virus and had glandular fever. He searched 3 years for a diagnosis. Went to every doctor there is: physicians, neurologists, internists, hematologists, even oncologists.
2/7 Everyone told him "Your blood and body is fine, it must be psychological." Only through sheer luck, a hematologist knew a professor at the university of Heidelberg, who then told him about #MECFS. But german doctors and authorities won't acknowledge the sickness.
3/7 He cannot work as a chemical technician at @merck_de anymore, because it is in shifts and physically too challenging for him. He was put in many different positions at the company, but none would fit, because his new managers couldn't cope with him.#MillionsMissing

1/🧵Real World #LongCOVID: listen up

Words from a #LongHauler (shared w perm): “It’s been a year, Dr. Wes, & my hand tremors are getting worse. I don't have them all the time, but some days, for some reason, my right hand & my legs want to just shake…

bit.ly/3afyE3k Image
2/ The tremors trigger me knowing it's going to be a tough day, so I try not to work on tasks such as like invoicing or complicated emails. If there's a fire I need to put out, I get someone to help me with it because my mind is just shot…

3/ Today is a good example. After I got out of the hospital, I got a lot better for a few months, but then the #LongCOVID stuff started and I’m declining. I’ve never left my phone at home when going to work, and now I’ve done it 3 times. Misspelling simple words,…

1/🧵 People ask me, “What’s the main purpose of treating #COVID ICU pts with the #A2Fbundle? (a daily safety checklist of ⬇️ sedation & ⬆️ mobilization). It’s to prevent #PICS related dementia, PTSD, depression & reduce suffering in #LongCOVID.

HOW? What is the philosophy?
2/ For patients it’s about magnifying their #dignity and self-worth by liberating them sooner from us (our treatments that put them through a depersonalization chamber). For HCWs, it’s about purity of service, avoiding hypocrisy, and better self-sacrifice for our patients.
3/ Waking people up and walking them w family at their side in PPE (even inside a room if #COVID+) allows them to feel alive again. Repersonalized! Human. Seen. Drives away #delirium & #brainfog. Wouldn’t you want this?

1/🧵🎥 “Doc, TIME is BROKEN.” ⏰ #BRAINFOG in #COVID19 is something like this stuttering second-hand on a hospital room clock with a low battery. Imagine if your brain stuttered like this! This video was taken by an ICU survivor who told me how the clock contributed to #Delirium.
2/ Now our @CIBScenter ICU survivor @SGFesquire sees how the clock not only contributed to his delirium but symbolized his brokenness & need of healing (pic w perm). Brain Fog presents as delirium when 1st sick & as cognitive impairment (memory + mental slowness) in #LongCOVID.

Ongoing Brain Dysfunction = major suffering. Let’s be patient with #LongHaulers. They look normal & are silenced like #MECFS pts. Patients, seek help. We will help you “unbreak time.” We do this through cognitive rehabilitation tools. #recovery

@NSU_INIM's Dr. Klimas also emphasized the need for separating
- hospitalized COVID patients with organ damage that is causing neuro symptoms after COVID
vs.
- #LongCovid which typically is in non-hospitalized cases

@satish_r_raj: pharmacological treatment for #POTS. Notes that non-pharm mgt (compression, fluid balance) are generally the foundation of management, even for the many pts who do also require medication as part of their mgt strat. #DysConf2020

Up to 70% of #POTS pts have low blood volume!! Not heaps of data supporting this historically, but theorised that hypovolemia may increase sympathetic tonicity & trigger hyperadrenergic state. Now have data supporting this in forthcoming study! #DysConf2020
