1/10 Brilliant visit to @NTeesHpoolNHSFT today. Some really good, on the ground, examples of how joined up and integrated health and care services benefit patients. Impressive work, particularly on the urgent and emergency care pathway, that deserves a higher profile!
2/10 Really enjoyed visiting the @NTeesHpoolNHSFT integrated discharge team today. Great co-located service working with @DurhamCouncil, @HpoolCouncil and @stocktoncouncil. We all agreed physical co-location is important to drive maximum efficiency and effectiveness.
3/10 Great conversation with the @NTeesHpoolNHSFT integrated care discharge(ICD) team on what was different compared to the other ICD teams I have seen. Two things...the really effective use and central role / leadership of therapists, in the entire ICD process....
4/10 Really impressed with way the Integrated Care Discharge team @NTeesHpoolNHSFT reach out to support frail patients as soon as they arrive in the Emergency Department - therapy team formally and permanently attached to the ED.
5/10 We often talk about importance of using & developing skills of allied health professionals (still not sure I like the term). Really impressed with way @NTeesHpoolNHSFT are pushing the boundaries here. Great to meet my first ever dedicated ED pharmacist, for example.
6/10 Often wondered why @NTeesHpoolNHSFT often tops table for 4 hour A&E performance, now I know! Seriously impressive integration and co-ordination across entire urgent and emergency care pathway. GP Fed, @NEAmbulance and @NTeesHpoolNHSFT lead and provide all services together..
7/10 Fascinating that 111, out of hours, Urgent Care, 999 and A&E services all being commissioned as a single whole and provided by a tight GP/ambulance/acute and community trust partnership produces best A&E performance in country. Some important messages for us all????...
8/10 Impressive management of urgent & emergency care patient flow @NTeesHpoolNHSFT. Careful thought on where to put capacity, how to set and manage access criteria to different elements and how to focus on providing right care for patient need, avoiding default to admissions.
9/10 Great presentation @NTeesHpoolNHSFT on Integrated Single Point of Access to community and social care services, with very effective joint working with @HpoolCouncil. Love presentations with real case studies - brings it alive! Thanks!
10/10 Great to see Julie Gillon & team going the extra mile @NTeesHpoolNHSFT to provide a joined up, integrated, health and care service for local population. Good to hear plans to further deepen and extend relationship with local GPs and work with emerging Primary Care Networks.
11/10 When worlds collide...Always been a fan of contribution of @PaulWilliamsMP to @CommonsHealth. Delighted to hear him talked of in very fond terms @NTeesHpoolNHSFT for his work helping set up integrated urgent and emergency care pathway before being elected as an MP.
