2/ I spent 50 mins with patient/family - discuss prognosis, comfort care - the works. Once patient left 4som blood tests, I tell family, separately, regarding end of life care, once the time comes. They wer sad but contend. Very crunched 4money. So best option - they cudnt afford
3/ Best approved option for advanced liver cancer is Atezo+Bevaci - too expensive. So they tuk next good option - Lenvatinib - affordable for them. I sent a detailed letter with plans, along with patient son, to the evidence-based turned #ayurveda oncology professor. #MedTwitter
4/ 5-days later, the son comes back to return all medicines including Lenvatinib - purchased from the pharmacy. He wanted me to sign on the bills for return. I did. I asked him Y? Was he going for ONLY supportive care? He said, no, oncology #herbalist said these meds wont work.
5/ And he wanted to try naturopathy with safe herbals on him for a month. Then if THAT did not work out, he can go ahead with what #evidence-based medicine (me). I was shocked. More than insulting me, it was more of immorality towards the patient/family. I knew it. #livertwitter
6/ The retired blood-sucker was in it for the money.
I hav seen his prescriptions before
Eg:
Where he nearly killed a breast cancer patient in remission with 10K INR / mo worth medicines.
I speak my mind to the son - he may take it well, he may not.
7/ He may think yes, I am talking sense.
Or He may think, I am out 4 money becoz all these expensive medicines are being returned.
I am a salaried man. I see 25 patients, 1 see 100 patients, I get my fixed salary every month. It wont change until the last patient I see in my life

1/I'm really not sure what you are getting at. You sound sweet, but ur intentions with respect to medical care are not ideal, SEEMS like u r trying to legitimatize quackery (not sure if u r an ayush practitioner-plz confirm)
This is not welcome and unacceptable behavior.
2/ What does it mean /in their own areas/ this is very concerning. Are you promoting quackery? Do u kno principles of AYU & Siddha. They are highly unscientific. Feels like U R somehow trying to speak pseudoscience without actually speaking pseudoscience - an #AYUSH sympathizer.

1/A horrific patient scene today. A #kidney#transplant patient doing well since 2015. Had fall and fracture of foot. Underwent surgery,improving. Asked for physio sessions to continue at home. Asks around,finds #ayurvedic traditional healer. He does external works.
2/ patient feels better. #Ayurveda healer gets his trust. Says post transplant infections and diabetes worsen (patient was already diabetic kidney disease for which transplant was done). Says #natural is safest way. Patient falls for healers confidence. Starts #herbal drugs.
3/ Drugs and potions prepared at home from locally sourced plants. Small fee Healer also advise nutritional changes. More like starvation. No proteins. Remember patient is post kidney transplant. #weightloss in 1 mo. Loss of appetite in 2 months. Then came something unexpected.
