Our new case study highlights why it's important for physicians to better understand the body's response to #exercise #training. 🧵1/5

🔓… #medicine #liver #physiology @theliverdr
In Oct 2021, I had a routine blood test that revealed elevated liver enzymes. A later blood test with a different doctor showed the liver panel was "elevated and worsening", more than double the normal range for some variables. 2/5
A series of additional tests, including abdominal ultrasound, were negative. Physician referred me to a gastroenterologist who suggested (an invasive) liver biopsy. However, before the next blood test, I stopped exercising for 7 days. 3/5

1. Coffee is a popular beverage & is known to have multiple health-related benefits including better overall survival in several diseases. A systematic review looked at association of coffee and #liver disease.
2. Coffee consumption was associated with improved serum gamma glutamyl transferase (GGT), aspartate aminotransferase (AST) and alanine aminotransferase (ALT) values in a dose dependent manner in individuals at risk for liver disease.
3. In chronic liver disease patients who consume coffee, a decreased risk of progression to cirrhosis, a lowered death rate in cirrhosis patients, and a lowered rate of hepatocellular carcinoma (liver cancer) development were observed.

I am a cardiologist, and my work place is above the diaphragm?! Why should I care about the #liver and #NAFLD (non-alcoholic fatty liver disease)?
⚡️Our therapies in many cases reach well beyond the heart
⚡️In order to improve outcomes we have to tackle comorbidities
🟦NAFLD (prevalence 25%) isn't only a hepatic disease but also a systemic & CV disease w/o effective pharmacotherapy
🟦We discuss the promising role of SGLT2i in NAFLD, propose mechanisms, & suggest future directions.…
The pathogenesis of NAFLD is multifactorial w/ parallel processes that involve insulin resistance, hepatic fat accumulation, increased lipotoxicity, mitochondrial dysfunction, oxidative stress, ER stress, abnormal autophagy, altered gut flora, & apoptosis.

1/As promised & after reading $NTLA latest Q1 2022 financial report here is my analysis regarding @intelliatx latest corporate status. I have focused only on the main issues that I found to be the most interesting & relevant. #CRISPR #GeneEditing #BioTech #FinTwit #Genomics👇
2/NTLA-2001 is the most advanced In-Vivo #CRISPR/#Cas9 #GeneEditing program in $NTLA portfolio & is aimed to treat Transthyretin Amyloidosis - (ATTR). NTLA-2001 could potentially halt & reverse the disease in a “one time” treatment by knocking out the TTR gene with a single dose
3/In February $NTLA has presented an updated clinical data from its Phase 1 study of NTLA-2001 which included 15 patients with ATTR amyloidosis. IMO NTLA-2001 readout looks promising & here is a summary that I wrote shortly after @intelliatx’s presentation of NTLA-2001👇

🧵 In the US, until recently, your chances of receiving a donated #liver w/in 30 days would vary significantly depending on where you lived—as low as 1%, or as high as 60%. Remember Steve Jobs' speedy liver transplant? He got on multiple waitlists...🧵…
For decades, experts and providers have been pressing for a more equitable system of organ allocation, one that would even the odds between, say, a patient in a dense urban center with a long waitlist and a patient in a more rural, statistically healthier area with a short one.
For almost 50 years, the system in place was an outgrowth of informal sharing networks between doctors and medical centers, resulting in a web of organ procurement organizations (OPOs) and donation service areas (DSAs) spread out across the country. #donatelife

Hey Twitter! It's been a while. I used to write here all the time hanging out with #scifichat, #comicschat, #litchat, talking about #science, #tech, #autism and a dozen other topics which broadened my horizons and let me share my diverse interests in almost everything.

4 years ago, she underwent an operation for #peritoneal #dialysis. This form of at-home dialysis would let her take care of her renal needs at home avoiding the need for external #hemodialysis centers. In hindsight, this may have saved her life.…

1/🧵🎥 MAJOR TIPS about #Liver Disease
Why do we ask pts to “hold up traffic”? When people get severe liver disease, “asterixis” is a finding on exam we often see.

What does it mean?
#MedTwitter #NurseTwitter #TipsforNewDocs #Addiction
2/ Asterixis indicates that toxins like ammonia have built up in the body because the scarred liver no longer filters them out. The brain cells no long function normally (#Delirium or Hepatic #Encephalopathy) & can no longer control muscle movements like posture!
3/ These same toxin cause vessels to dilate and develop problems with blood flow in the skin and lungs. A skin finding is called “spiders.” They look like a multi-legged spider, right? It’s a tip-off.

Real, not fiction. Even though may sound like super fiction. Of a patient now admitted under me.

56 yr man. Prior #smoker No #alcohol use, no #DRUGS. Family guy. Starts havin cough. Nagging no sputum
Keeps him awake at night
He goes 2 a GP. Clinical examination and blood tests done. Says some chest infection. Puts him on antibiotics for a week. Asks for review. 1wk later some change in symptoms, but not fully ok. GP extends antibiotic course 7 more days. C, no fever and no signs of infection yet.
After 14d course, no change. Patient chucks the GP. Goes for a senior GP. Senior GP does clinical exam, blood tests AND chest X ray. X ray comes back as "not looking normal" 🙄 But no diagnosis. So he says. Let's try a short course of #antibiotics AND antifungal. Again 14 days
