Case of a man who shaved only on right side of face
1. 70-year-old man was brought to OPD three months ago by his son, with a complaint that he had stopped shaving on left side of face for one week. He was also not using his left hand even though he had no weakness in that hand.
2. This abnormal "behavior" started all of a sudden.
When his family members pointed this oddity, he retorted by saying, the left side of face did not belong to him.
Even when he was presented with water or tea from left side, he would stretch his right hand to receive them.
3. Family members were taken aback by his "odd behavior" and decided to seek medical opinion.
His BP was 170/110 mmHg and blood sugar was normal.
He had no known medical illness. #MedTwitter #neurotwitter
4. Examination revealed normal strength and sensations in his limbs and face. His vision was also normal.
He could not perceive objects or people standing on his left side.
On showing his left hand to him, he remarked "the hand is not mine and belongs to someone else".
5. It was now clear that he was suffering from left-sided #hemineglect.
This neglect affected his own body parts on left side as well the surroundings/environment on his left hand side.
Suspecting a disease in right side of #brain, #MRI brain was ordered.
6. MRI Brain showed #infarct (blood clot) in right parietal cortex. This could fully explain his left hemineglect.
He was started on anti-platelets, statins and BP-lowering drugs.
He was also advised #rehabilitation.
At a recent follow up, he had shown considerable improvement
7. Take home message *Neglect of own body parts or surroundings on one side (usually left side) is caused by a disease in right side of brain.
*If symptoms start all of a sudden, it could be due to #stroke.
Rapid diagnosis and prompt medical treatment can help in quick recovery

1. A longitudinal study (2007-2021) was conducted on 27,000 people from 32 countries belonging to various continents to assess the relation of self-reported stress and risk of various types of stroke

2. Increased stress at home and at work (within the previous 12 months) increased the risk of stroke by 95% (1.95 times) and 170% (2.7 times) respectively, as compared to no self-reported stress. Recent stressful events increased the risk of stroke by 1.3 times.
3. This association was consistent for all stress domains, including work stress, home stress, and financial stress. The association was independent of socioeconomic status, occupation, and educational level & remained significant after adjustment for cardiovascular risk factors.

Case of a woman whose shoe size increased from 5 to 7 over two years

1. It was an usual OPD, when I was counselling one of my patients with back pain about exercise, diet and correct posture. Incidentally, I noticed his wife's face who was sitting beside the patient.

2. Her nose & lips appeared a bit larger than usual. When she smiled, her teeth were little spaced out and tongue was also little larger than normal. She spoke with a little heavy voice.
Being a #neurologist, I was imagining the possibility of a brain #disease.

3. I asked her- "Don't mistake me, but has your shoe size increased?" Her eyes almost popped out. She replied with a shocked expression- "doc, yes! It has increased from 5 to 7 in the past two years. Don't everyone's feet become larger as they grow older?"

Sleep more to lose more weight

1. Yes, you heard it right. Researchers found that overweight individuals could lose significant amount of weight by just sleeping for longer duration.
Short #sleep duration is a known risk factor for obesity.

2. Adults aged 21-40 years, with BMI 25-29.9, with habitual sleep duration <6.5 hours/night were included in the study.
All participants were instructed to continue daily routine activities at home without any prescribed diet or physical activity.
3. Participants were randomized to either an individualized sleep hygiene counseling session to extend their bedtime to 8.5 hours (sleep extension group) or to continue their habitual sleep (control group).

Elevated #Homocysteine levels increase the risk of #stroke

1. Homocysteine (Hcy) is a sulfur-containing amino acid produced by the demethylation of the essential amino acid methionine, and the normal level is 5-15 µmol/L. High Hcy levels can increase the risk of stroke.
2. Total Hcy elevations above 15 µmol/L are an independent risk factor for ischemic stroke.
The risk of stroke is two times higher in people with Hcy >15 µmol/L as compared to those with Hcy level <10 µmol/L.

3. The risk of #death is 43% higher in patients with ischemic stroke, who have elevated Hcy levels.
Hcy is not only a risk factor for, but is also correlated with, stroke severity, worse #prognosis, and stroke recurrence.

Green Mediterranean diet (GREEN-MED) lowers visceral adipose tissue

1. Standard Mediterranean diet (MED) is a healthy diet option and causes weight loss in obese. In a study, GREEN-MED diet was compared with MED diet, over a period of 18 months (both groups exercised equally).
2. Both the green and standard Mediterranean diet groups achieved similar weight loss (–6.2 kg and –5.4 kg) compared with the standard healthy diet guidelines (HDG) group (–1.5 kg).

1. This seems to be true.
A recent extensive review of studies done in geographies with maximum 100+ aged people showed that most of them practiced some form of calories restriction (CR) right from childhood.

This can be achieved by either intermittent fasting, or time-restricted feeding or substantially reducing carbohydrate intake.
