So recently I did a FOIA (Freedom of Information Act) request regarding CMS radiology (MRI) safety standards at outpatient imaging centers.

The results were disturbing...
The 2008 MIPPA act sect. 135, (3) provides that the "Secretary [of HHS] shall establish procedures to ensure that the criteria used by an accreditation organization…"
...and "standards that require the supplier have procedures in place to ensure the safety of persons who furnish the technical component of advanced diagnostic imaging services and individuals to whom such services are furnished"

Team it's time to address the Hoeg et al preprint on post-COVID vaccine myocarditis. How am I qualified to speak about myocarditis? I am board certified in paediatric cardiology and currently have patients recovering from this condition. TLDR: They are doing quite well. #MRI
What is the premise of that article? directly scanning VAERS data will result in capturing more cases of myocarditis, and therefore the "conclusion" that COVID vaccine generates a higher risk of myocarditis than COVID disease. But - cardiologists have diagnostic criteria. #heart
All that bleeds troponin is **not** myocarditis. I cite my professional society, the American College of Cardiology: #myocarditis #COVID19 #vaccination #VaccinesWork Image

Welcome to MRI Safety Week!

All this week we're highlighting various 'why' elements of MRI Safety Week and the Year of MRI Safety.

Today's 🧵: What has changed in MRI safety?

#MRI #MRIsafety #MRIsafetyweek
Immediately following the Colombini accident (see yesterday's thread), the @RadiologyACR convened what they called a 'blue ribbon panel' on MRI safety. That group published the first MRI safety best practice manual in 2002.
That specific document was expanded and re-released in 2004, 2007, 2013, and most recently in 2020.

The UK's @MHRAgovuk released MRI safety guidelines in 2007. Also in 2007, the @RANZCRcollege released their MRI safety guidelines (borrowing heavily from the ACR's publicaitons).

20 years ago -almost to the minute- a young boy was being taken to the MRI of the county hospital.

A few days prior he had an accident on the playground and a CT scan of his skull revealed a brain tumor that would likely have killed him, had it not been found.
The boy was transferred from the community hospital to the larger county hospital, where the tumor was confirmed, and brain surgery was planned.

The surgery was a success, and all indications were that the tumor was fully removed and the boy would have a full recovery.

@Wagner_Nephro @sennasthunder @lambertdebb My observation: Hard strings extruded through scars to create long Strands like Hairs, some like ceramic knives. If I pluck a shard from left thumb tip, I feel a tug in my neck, shoulder. Once the hard pieces come out, completely healed.
I thought with all my tugging and unconscious biting of my fingers and several old scars that they would be Red, Inflamed, Bleeding, Infected. They are not.I thought if I had OCD that my fingers would never heal. They are healing once the chunks are out. Sunlight, salt, charcoal.
Just realized I stop telling my Doctors.
They just give me a roll of the eyes 👀 and maybe a little internal chuckle 🤭 as they Observe Old Nurse Mary looking fine as ever. Never showing pain so bad it causes nausea. Yup, that’s part of our problem, pwMS 🤦‍♀️ @GavinGiovannoni

@US_FDA approved the #Qcollar jugular compression device to "protect athletes' brains during head impacts."

This decision was based on VERY FLAWED DATA and does NOT demonstrate the safety or effectiveness of the Q-collar.

Here is the text from the @FDADeviceInfo press release.

If you don't understand the highly nuanced #MRI technique known as DTI, these results sound straightforward and convincing.

THEY ARE NOT. Don't be fooled by these numbers!🤔

I will dissect these in the thread below. Image

"No significant changes" is based AVERAGE response, not individual.

First, an easy-to-understand analogy below.

If half the sample experiences an increase and half experiences a decrease, they can cancel each other out to falsely suggest "no change" when one does exists! Image

Do the benefits outweigh the risks in using MRI gadolinium-based contrast media (GBCM) in patients with kidney disease?

MAJOR new consensus statement from @RadiologyACR and @NKF published in @Radiology_RSNA at…

A #RadInTraining #TWEETORIAL thread 👇
GBCMs consist of gadolinium ions chelated with organic ligands to minimize toxicity from free gadolinium.

Which chemical class has a lower rate of free gadolinium dissociation?
Answer: Macrocyclic GBCMs are more stable (lower ratio of free gadolinium to complexed ligand). (Remember, 𝐋inear is 𝐋abile)

Macrocyclics form a complete ring, whereas linear agents form an incomplete ring.

You're invited to join in the recognition of MRI Safety Week from July 27 (the anniversary of the infamous Colombini accident) through to August 2.

#MRIsafety #MRI #Radiology #PatientSafety

Some of you may be thinking, 'hey, isn't MRI the "safe" imaging choice? We're supposed to worry about X-rays and ionizing radiation, and move to MRI when possible, right?'

Well a lot of elements baked into that conventional wisdom have been shifting over time...

Let's start with medical ionizing radiation exposure (predominantly experienced as X-rays). Concern about medical radiation exposure, and resulting technological advancements in imaging equipment, have substantially reduced per-exam patient exposure.

Rapid and widespread changes in #brain anatomy and physiology in the first five years of life present substantial challenges for developmental #MRI studies. One persistent challenge is that methods best suited to earlier developmental stages are suboptimal for later stages
This new review describes the data acquisition, processing, & analysis challenges that introduce these potential biases when conducting & analyzing data from infants and young children & attempts to elucidate decisions & recommendations that can optimize developmental comparisons

Now eye enjoyed an important question addressed at #EAN2020. Good to think about when you are called to ED for #opticneuritis. Let’s think about it together. Let’s focus on #clinicaldiagnosismaking #meded #medstudenttwitter #medtwitter #neurologyresident @WNGtweets @EANeurology
How do you tell if it’s optic neuritis or not? #painless #unilateral #altitudinal -means top half or bottom half of the field - vision loss more likely to be non-arteritic AION. Any other red flags less likely to be optic neuritis?
Now if it’s #painful it’s more likely #opticneuritis - but what sort? Bilateral vision loss and painful, think #MOG. Unilateral severe vision loss more than pain, think #Aquaporin4. MRI orbits show longitudinal extensive optic nerve enhancement in both. So MRI orbits for clues.

Then you think - active or inactive process? If it’s actively destroying the BBB, it’ll enhance on contrast enhancement. So get MRI with and without contrast. Why without? T2 shine-through - but that’s for another time. #MedStudentTwitter #neurology #neurologyresident

I am very excited to share our latest work published in @eLife! We combined #MEG, #fMRI and #MRI for #BrainAge prediction & #biomarker development. Each modality added unique information & enhanced brain-behavior mapping! Thread👇
1/ We combined anatomical MRI (surface area, thickness, volume), fMRI-connectivity and MEG (source power, connectivity, alpha-peak, 1/f, latencies) with a stacking approach (1: ridge, 2: random forest). We made sure our model potentially extracts information from missing values.
2/ Is combining multimodal brain data worth the effort for brain age prediction? Compared to anatomical MRI, fMRI and MEG added similar improvements. Combining all modalities yielded to markedly improved prediction performance. MEG and fMRI contributed independent information!
