I've had several conversations in the past week with people trying to understand the clinical trial recruitment space. Some notes:
The reason recruitment is low for clinical trials is not JUST because patients don't know that the trials exist, which is what I think most people assume

Many are excluded because of really stringent rules around excluding them, or it's inconvenient to be a part of the trial
Also, the rules around which patients are not allowed to be in a trial are pretty specific, so for a patient to be a good fit for a trial you actually need a lot of data about them (diagnosis, prescription history, etc.)
If you're trying to sell clinical trial recruitment leads, you better had a lot of information about the patient's health history and/or have a very engaged patient base
Also it's hard to identify a lot of patients because there's certain data you need about them that's not strictly coded anywhere (EMR, Claims, etc.) - this is why you often have to work very closely with providers to get that data/flag patients
And this is important too - in many cases there's a ton of reasons (as said here) that would prevent a patient from being a part of a trial OR actually STAYING in a trial

Several people brought up timing - catching a patient at the exact right time in their disease cycle is very very difficult when most of this data ends up arriving not in real-time
