The Ass & the Load of Salt

A Merchant, driving his Ass homeward from the seashore with a heavy load of salt, came to a river crossed by a shallow ford. They had crossed this river many times before without accident, but this time the Ass slipped and fell when halfway over.
And when the Merchant at last got him to his feet, much of the salt had melted away. Delighted to find how much lighter his burden had become, the Ass finished the journey very gayly.

Next day the Merchant went for another load of salt.
On the way home the Ass, remembering what had happened at the ford, purposely let himself fall into the water, and again got rid of most of his burden.

The Shepherd Boy & the Wolf

A Shepherd Boy tended his master's Sheep near a dark forest not far from the village. Soon he found life in the pasture very dull. All he could do to amuse himself was to talk to his dog or play on his shepherd's pipe.
One day as he sat watching the Sheep and the quiet forest, and thinking what he would do should he see a Wolf, he thought of a plan to amuse himself.
His Master had told him to call for help should a Wolf attack the flock, and the Villagers would drive it away. So now, though he had not seen anything that even looked like a Wolf, he ran toward the village shouting at the top of his voice, "Wolf! Wolf!"

OIL is taking a beating today due to Russia not agreeing to the production cuts OPEC wants. Screwed up the oil stocks.
Dow off it's lows now down 400pts.

2/5 The current state of diagnostic testing for #Coronavirus is expanding rapidly/ widely- all 50 states now have testing capability. This will support the ability for practitioners to diagnose #COVID19 as community transmission occurs in some areas at this phase of the epidemic.
3/5 In addition to state public health labs, testing will soon be available at many academic medical centers & private sector. @US_FDA has facilitated use of these tests in patients by an Emergency Use Authorization. @CMSGov has created new coding to facilitate reimbursement.

THREAD: Getting reagents to labs to test specimens is one part of challenge. That's the 1.5M "tests" discussed in article. Means there will be enough reagents sent to public health and some academic labs to test up to 1.5 million specimens. Great news 1/n politico.com/news/2020/03/0…
The other part of the challenge is getting more labs operating the #coronavirus test. If your only lab can prep and test 100 samples a day, your "throughout" is only 50 patients that can be tested a day (2 samples per patient). More reagents won't get more patients tested. 2/n
So the other part of the challenge is getting more labs into the effort. This is where the efforts by Quest and LabCorp to start their own testing, as well as the FDA policy accommodation that makes it easier for academic labs to start running their own tests, is critical. 3/n

The current case fatality rate (CFR) for #coronavirus is 6%. For every 100 closed cases of coronavirus, six people have died globally.

That makes it 60x deadlier than the flu, which kills 0.1% of those diagnosed.

The coronavirus is extremely contagious. Some estimates for R0 range up to 8 or even 9. That’s 6x the contagiousness of the flu.

Multiply 60x the mortality times 5-6x the contagiousness and you get that coronavirus can kill more than 300x what the flu does every year.

One of the many questions of the past week in the US has been: "WHY AREN'T WE TESTING ALL SUSPECTED CASES?" This is a very fair and natural question if you do not work in a clinical laboratory or, more broadly, in R&D as applied to human health. 2/
To set the stage, imagine if there would be mass disruption to everyday life by the triggering of containment policies on a grand scale, not to mention lives on the line, if absolutely everything at your work was not reliably perfect every time. Those are the stakes. 3/

#coronavirus is a story of squandered time. This story here is perfect example. Our planning already had in place protections for first responders. And we had time. China gave us time. And we squandered it, failing to protect because there was no one to demand it or execute it.1/
Lost, squandered time will be how history writes about our preparedness. That is why there is this uptick in panic and surprise. The WH continued to hope and insist that China would succeed at containment. That was plan A. But we have a plan B for a reason. 2/
How do I know it was part of plan? Because we’ve done this before. The virus is new; pandemic preparedness is not. We knew border agents and other first responders would need additional protections; same true for public health workers. (See this thread on the plan ⬇️). 3/
