Good news everyone: My insurance provider denied coverage for all my CPAP supplies. I now owe them $330. Here's their thorough explanation: 
They did this in order to steal from me. They robbed me of $330 unless I pay in time, stress, and arguments that I'm not emotionally equipped to make. They did this on purpose. The above messages are there so I'll be forced to call and be read a line of horseshit.
They did this because they know I probably won't call, because most people don't call, because everyone knows what Kafkaesque means and everyone knows you can't win. It was denied because they want me to pay them money.
It's a fucking CPAP. There is no acceptable reason to deny coverage, I need this equipment. My stress level is now through the roof. I shouldn't have let them bill insurance. I should have just paid out of pocket and left.
A while back I went to the dentist. He said "We should do fillings on these three teeth." I said, "Do just one." He looked at me like I was out of my mind. I had to repeat the request and explain it four times.
"My employer switched insurers. If you do three, I'm going to be $800-1000 in the hole. They will probably deny coverage, and I can't afford that." I convinced them to do just one. Insurance covered 50% after fighting with them twice. I would have had a $500 or $1000 bill.
I am uninsured. I have no meaningful health insurance. I have no doubt at all that if i went to the ER I would be in debt for the rest of my life.
Why even namedrop the company? They're all the same. They are criminals, in cahoots with the US government. This is not in dispute. There is literally nothing to discuss. This is an outright fact.
The US government guaranteed them business, the healthcare industry engaged in a massive price fixing scheme in order to guarantee them business, and now they won't let go of a single dime. They never will. Never again.
America has no healthcare. If you get sick here you just die.
