Omar makes 8 specific claims in this thread. 6 of them are false or very misleading.

The problem isn't that she's consistently dishonest, but that the press takes no interest in correcting her dishonesty.
To go through them:

1) Toddler in cage image was from protest: snopes.com/fact-check/tod…

Yes, kind are being kept in facilities with chain-linked fences, just as thousands were during the Obama admin (thus why many of the pics are from 2014).
2) Blaming DHS, and Nielsen specifically, for the death of that girl is beyond dishonest:

&
3) "Repeatedly lying about taking children from their mothers"

This one is more of an opinion. She said several things that are definitely arguable as false so I am willing to give Omar this one.
4) "He was in charge when CBP fired tear gas on a crowd of migrants seeking asylum"

Clearly misleading. The tear gas was used to break up a rush on the border:

Was used in similar cases by previous admin:
5) "He failed to disclose details about the death of a child in CBP custody to Congress"

This is a complain over him not disclosing it in testimony he had before Congress, but he wasn't asked about it and it wasn't relevant to his testimony at the time.
6) "the President is calling asylum seekers 'animals''

This is indisputably false. Based on a viral tweet that has been repeatedly debunked and even source has withdrawn it.
7) Saying the country is "full"

You can argue over context and what exactly Trump meant, but this comment is mostly accurate. realclearpolitics.com/video/2019/04/…
8) "The acting DHS Secretary commanded the CBP officers who physically separated over 2,800 children from their parents."

What does that mean? He followed the DHS policy and did his job? It was not his policy. Clearly misleading.

Like I said, 6/8 are false or misleading.
