How we should watch out that we can be insensitive even without meaning to.

Health personnel, med students included, get frustrated when patients go to the ER rather the OPD for non-urgent conditions

or when patients go to govt hospitals for conditions that should be seen at the health center.

And with too many patients, this can be very taxing. 1/n
Hence the all too common "Bakit ngayon lang po kayo nagpatingin?"

More than anything, this is a rhetorical question, because truth to tell

1) there is judgement in that question
2) the answer has no bearing on the management
3) the patient does not need to justify anything. 2/n
"Bakit ngayon lang kayo nagpatingin?" is commonly preceded by "Matagal na pala yan..."

and worse, frequently uttered in frustration or sometimes, with disdain.

But is this really necessary? Do patients need to hear this, because this is NOT patient education. Really. 3/n
Patient education is important, esp because patients have a different concept of "emergency".

But more than this, understanding context is even more essential.

Under the current social conditions, there are just so many factors in the decision of patients to seek consult. 4/n
Access is not just a matter physical presence or geographic proximity.

Pag nag-absent ako, wala akong kikitain. Wala kaming pangkain.

Walang-wala ako. Kailangan pa umutang. Magkano kaya gagastusin ko?

Or worse,

Baka pagalitan na naman ako ni Dok kasi di ako agad nagpunta. 5/n
A woman who brought her husband to the ER was berated by the doctor "Bakit ngayon nyo lang dinala? Binintay nyo pang lumula."

"Hindi ko masabi na hinintay ko pang sumweldo yung anak ko." she told me.

Her husband died.

"Sino ba naman ang ayaw magpatingin agad kung kaya?" 6/n
In an ideal setting, the ER should only be for those who need urgent care. This is true.

But our healthcare system is very far from ideal.

The social determination of health account for many considerations, esp for the poor.

THIS is the context that we need to appreciate. 7/n
Our healthcare system is already failing our people.

Let us not add to this injustice by failing them again when they actually do get to us, when they reach our instititions.

Understanding and compassion, now more than ever. 8/8
