In response to many requests, I've collated several #COVID19 cases at one of my hospitals over the past 5 days. In this limited (non-peer reviewed) cohort:
- 100% subpleural involvement, 33% + centrolobular involvement
- 40% even apical/basilar, 27% basilar dom, 5% mid dom. 1/17
- All have ground glass, 20% with atoll, 2 with ARDS
- 66% had some + CXR finding I could correlate to a CT finding
- 66% had documented exposure, rest I couldn't find either way

1/Week 2 of #COVID disaster response here in Seattle. King County has reported 420 cases and 37 deaths. We are seeing more volumes in our hospitals, though I think we it may be the tip of the iceberg and anticipate more to come.

A couple of additional lessons this week:
2/1. Your PPE supplies are directly related to the time it takes to rule out COVID19. @JohnLynchID has denoted this the “MAK unit” (named after the inimitable @molsenmoose) and it represents the time from initiation of PPE for a patient to negative test and de-escalation.
3/ It is CRUCIAL. A large amount of your patient volume initially will be rule-outs and being efficient here will save your resources.

Low "MAK" time allows you to have low threshold to test inpatient and keep your care moving forward.

Over the past few weeks I’ve fielded countless questions from worried parents and families about #CoronavirusUSA and #COVID. The amazing thing is most families have not been panicking but moreso just want their bearings. How worried should they be? What keeps kids safe? 1/
The first thing I’ve found is to acknowledge the fear. This is scary and there’s a lot of confusion and misinformation. That’s okay. Doctors are here to help and it’s okay to ask. The second is to not panic. People should be concerned, aware, but not panicked or dismissive. 2/
Next give reference points. This isn’t World War Z. It isn’t Ebola with its >50% mortality but it isn’t flu (as many say) with ~0.1%. Lots of data says ~2% but that’s probably a bit high because we’re testing so few. It depends on risk factors. Regardless don’t bulk buy TP. 3/

Global Case Fatality Rate (CFR): 3.5%
[(3800 deaths / 110,000 confirmed cases) X 100]

1. CFR will come down as more people are tested.
2. Unlike flu, CoVID19 has to be tested. Millions may have it but recover without having gotten tested, i.e. CFR may be inflated.

#COVID19 Flight attendant for Korean Air dx'd in Korea. She flew flights in/out of LAX/Seoul as well as Tel Aviv/ Seoul.

Also believed she walked Los Angeles Koreatown prior to return flight to Seoul.

Source Korean news &South Korea's Center for Disease Control.
This also on the heels of US military confirmed case in Seoul yesterday.

Many of you shared or read the VICE article '‘High Likelihood of Human Civilization Coming to an End’ Within 30 Years’ - based on a report from Breakthrough National Centre for Climate Restoration, an Australian think tank.

This was misleading & needs some caveats
1. "The "report" is not a peer-reviewed scientific paper. It's from some sort of "think tank" who can basically write what they like," says Prof. Richard Betts of the Met Office Hadley Centre & University of Exeter

This happened similarly with Jem Bendell's 'Deep adaptation: a map for navigating climate tragedy' paper (2018), which prophesised "inevitable", "near-term" social collapse -- & got hundreds of thousands of downloads.

Rejected by peer reviewers, who called for "major revisions"
