Social Prescribing is a key part of our Comprehensive Model for #PersonalisedCare alongside: Shared Decision Making, Patient Choice, Personalised Care and Support Planning, Self Management Support, & Personal Health Budgets
We will be publishing a definitive delivery plan on Thursday detailing how we will scale #PersonalisedCare over the next 5 years to support 2.5m people - including 900,000 people who will benefit from #SocialPrescribing
Link Workers will support GPs by spending time with people, finding out what matters to them, working with them to develop a plan and supporting them to connect with community based activities
Whilst some people already benefit from connections to their communities & have the confidence to seek out activities for themselves, many need assurance & support to find activities that are right for them
The expansion of #SocialPrescribing is not about replacing other forms of support already in place across health and social care but about working alongside these to create new channels for improving health and well-being
We have worked with people and communities across the country to develop best practice models for delivering #SocialPrescribing - needs strong local relationships and cross sector working to deliver maximum benefit for people and systems
