[1/11] Nonunion teaching case. These are my most challenging/frustrating but rewarding cases. Injured 5 months ago—not quite 9 month FDA definition but fit Brinker (“unlikely to heal without intervention in surgeon’s opinion”). Leg floppy, varus and externally rotated 90 degrees.
[2/11] Although this seemed like clear case of undertreated hypertrophic nonunion with stability issue, CRP slightly elevated so I did 2-stage. First was bone biopsy and cultures, which were negative. It also showed how big metaphyseal gap is once hypertrophic bone cleared out.
[3/11] 2nd stage involved standard lateral approach to proximal tibia, and medial approach at the “spike.” Bone cleared out both ways and here you can see the attempt to mobilize things. It doesn’t work completely; that happens when soft tissues contract after long time in varus.

[1/17] A story. The talk about death yesterday reminded me of my patient from years ago. Mary (not her real name) was 95, in great health, active in the community and all mental faculties at full strength. She fell and had a hip fracture for which I had to do a hemiarthroplasty.
[2/17] Depending on the study, hip fracture mortality rates are 20-40% at 1 year, usually from heart issues. That’s why we call it a “sentinel event,” and the death generally has nothing to do with the surgery; hip fracture is an indicator of frailty. But I thought, not Mary.
[3/17] She busted up into the clinic 2 weeks from her surgery (when most people have a hard time getting out of bed)... wearing heels. And no cane. Just waltzed in, telling everyone how great I was and how I had replaced her hip. Some people really do have a magical constitution.

[1/6] When I interviewed for BA/MD program at age 17, I sat in the lobby with a bunch of genius kids who were all talking about their biochem research. I was a kid who wrote stories; fanfiction to be exact. I figured there was no way I’d be getting in, so might as well have fun.
[2/6] The interviewer surprised me by asking about my stories. In previous Ivy schools etc, it was obvious they just saw my application that day, but it was clear this guy had actually read it. I wasn’t sure why he was asking; I had expected to talk about grades and science.
[3/6] We had a nice chat about Harry Potter. Because I wasn’t trying to impress him, I wasn’t even formal, doing stupid stuff like slapping the table and laughing like an idiot. The program took 5 out of 600 applicants, and I thought, ‘yeah, no way in hell.’

For the better part of 47 years my dad wore a silver POW/MIA bracelet with Knight’s name engraved on it. These bracelets…
…These bracelets were sold as a fundraiser so POWs and those missing in action would not be forgotten. My dad bought his from his sister, my Aunt Kathy Kaczmarczyk, in ~April 1972 in our hometown of Burlington, NJ.

He wore it every day as long as I can remember.
He told me that the bracelets were meant to be worn until they – or their remains – came home to America.

In 2002, he was working at Lowes and the bracelet caught on a shelf and broke. He placed it on top of our fridge so he’d see it every morning.

So Camdon Wright's Kickstarter for One Child's Heart (which I'm not connected to) has stirred up a fair amount of criticism... largely from white folks. And it's part of an unfortunate pattern, where white folks mobilize against a Kickstarter by a PoC creator bc "social justice"
I saw the same thing happen with @LulaVampiro and the Kickstarter for Thousand Arrows. People jumped all over that one because they were "concerned" about "cultural appropriation". EXCEPT. Where was that criticism for the 3000 other games about samurai written by white dudes?
