Do night shift duties pose any risks on health?

1. Large number of people are employed in jobs that require working at nights and sleeping in daytime.
Is it healthy? Are there any health hazards associated with this routine?
Let's look at some of the research on this topic.
2. Type 2 #diabetes :
Night shift work, especially rotating shift work including night shifts, is associated with higher type 2 diabetes odds.

3. Hypertension: In a recent study, night shift workers were found to have a higher #hypertension risk than day shift workers, which increased with increasing frequency of night shift work. Ref: doi.org/10.1016/j.mayo…
4. Stroke: Rotating night shift work is associated with a 4% increased risk of ischemic #stroke for every 5 years. The risk is more for #women and greater in women after shift work for 15 years or longer. Ref: doi.org/10.1093/aje/kw…
5. The risk of atrial fibrillation and coronary artery disease is higher among shift duty and night shift workers.
6. Cancer: Various studies on incidence of #breast and prostate #Cancer incidence have shown that the cancer risk is higher by 1-32% among those who do shift duties including night shifts, as compared to those who do not do night shift duties. Ref: doi.org/10.1136/bmj.i5…
7. Obesity: Night shift workers have almost 3 times higher association with abdominal #obesity than day shift workers.
Night shift workers sleep less hours, have higher weight, body mass index and abdominal circumference when compared to the day shift workers.
8. Depression: The risk of #depression is 33% higher among shift workers.
Females bear greater brunt. Female shift workers are 73% more likely to experience depressive symptoms than female non-shift workers. Ref: doi.org/10.2105/ajph.2…
9. #Accidents: People working longer hours or shift duties especially night shift duties are prone to cause accidents both at workplace as well as outside (while driving). This may be partly related to sleep loss associated with shift duties.
10. #erectiledysfunction: Working in night shifts and sleeping in daytime is associated with poorer erectile function. This is partly related to lower #testosterone levels (testosterone production peaks in REM sleep). Ref: doi.org/10.1016%2Fj.js…
11. #Death: Compared with daytime workers or those never exposing to night work, the risks for all-cause, cardiovascular and cancer mortality were 6%, 15% and 4% higher for those ever exposing to night work, respectively. Ref: doi.org/10.1016/j.slee…
12. Conclusions:
Shift work and night shift duties may be a necessity in many sectors. However, one needs to keep in mind the several health hazards that are associated with shift duties.
*Whenever possible, shift to daytime duties.

*Providing for a nap time during night duties can mitigate some of the health hazards.

T3 testing is not routinely needed in managing patients with primary #hypothyroidism

1. Primary hypothyroidism is a common hormonal disorder, which is treated with levo-thyroxine (T4) replacement.

2. In order to assess the adequacy of T4 dose, some clinicians routinely order #T3 along with #T4 and #TSH (thyroid stimulating hormone). Is routine T3 testing really needed in cases of primary hypothyroidism?
3. Clinician would be able to determine the adequacy or over-replacement of levo-thyroxine (T4) dose on the basis of T4 and TSH levels.
*In cases of over-replacement, T4 will be elevated and TSH suppressed.
*In cases of adequate dosing of T4, TSH and T4 levels would be normal.

Learning case for #doctors
1. 76-year-old man;
8-month history of statis #dermatitis, weakness and painful dysesthesias in the hands and feet.
Sensory-motor type #polyneuropathy was evident in a stocking-and-glove distribution, confirmed by nerve conduction studies.

2. Extensive testing including brain magnetic resonance imaging, spinal magnetic resonance imaging, and cerebral spinal fluid examinations were nondiagnostic.
Subsequently, serum vitamin B6 (VB6) levels were found to be low at 3.5 ng/mL (normal values: 6.0-40.0 ng/mL for males).

Longer daily #sedentary time increases the risk of #Knee, #Hip and #back #pain

1. 20-25% of people aged 50 or more suffer from orthopedic problems such as knee joint pain, hip joint pain, back pain and knee stiffness. Sedentary lifestyle is a major risk factor for them.
2. In a recent study, men with sedentary time of 7.5 hours or more/day had 45% higher risk of orthopedic problems as compared to those with daily sedentary time of <7.5 hours.

2. In a recent meta-analysis, the usage of hearing restorative devices (hearing aids or cochlear implants) by participants with hearing loss was associated with a 19% decrease in hazards of long-term cognitive decline.

To be beneficial, hearing aids should be worn for a minimum 4 hours per day.

A man who would become unconscious after singing romantic Kishore Kumar's Songs
1. 6 months ago, a 45-year-old man visited me for evaluation of a fainting episode that occurred while he was singing for a private gathering. He was a schoolteacher and singing was his passion.
2. On the fateful day, he had uneventfully sung a few songs of other singers.
On public demand, he sang “Pal pal dil ke paas”, a popular Kishore Kumar's song. A few minutes after completing the song, he collapsed on the stage and became unconscious.
3. His pulse rate was 110 beats per minute and BP was 160/100 mmHg. He was shifted to a hospital for evaluation.
He had regained consciousness while being taken to the hospital.
ECG, Echocardiogram and Cardiologist evaluation were normal.
