Excited to share our new paper out at @NatImmunol today! We discovered the limiting factor for #brain #Tregs is #IL2, and developed a new #genetherapy to treat #traumaticbraininjury and other #neuroinflammatory pathologies. 1/12

nature.com/articles/s4159…
The work started more than 10 years ago, after my brother Russell died following a #traumaticbraininjury. The more I read into it, the more treatable #neuroinflammation seems. Obviously there is a #brain delivery problem, but stopping #inflammation is basic #immunology! 2/12 Image
We had a great post-doc, @EmanuelaPasciu1, drive a project showing #Tcells in mouse and human brain, with key functions. Among these T cells were a small population of anti-inflammatory #Tregs, again in mouse and human. 3/12

“Good masks will keep you safe,” he says, referencing N95 masks.

Today, @AsthmaIreland made a public statement in support of Minister @EamonRyan's proposed Solid Fuel Regulations. We called on all parties to support the regulations in a bid to end the 1,300 premature deaths caused annually by fine particle emissions from burning solid fuels.
The new regulations would ban smoky coal and wet wood, and prohibiting the commercial sale of sod turf, will save lives, but provide for continued turf extraction by those with turbary rights and distribution within small communities. @Dept_ECC

1- My current take on whether anti-amyloid therapies are likely to have meaningful impact on cognition, functioning & quality of life of people living w/ #dementia due to #Alzheimers disease:

Not likely.

For details, based on my recent Grand Rounds at Penn St, read on.
2- History:

After finding that active immunization with Abeta42 in mice that overexpressed APP resulted in virtual elimination of amyloid deposits, there was great enthusiasm for anti-amyloid therapies - and for the amyloid hypothesis.

pubmed.ncbi.nlm.nih.gov/10408445/
3- More history:

Unfortunately, active immunization with Abeta42 in humans resulted in unacceptable side effects.

Passive immunization with various anti-amyloid monoclonal antibodies failed in multiple studies.

It seemed like this was the end of the line, until ...

At the age of 30, Georgie never expected that my dad might not know her name.

Her dad was diagnosed with dementia at just 46-years-old, this is her story.

🧵👇

metro.co.uk/2022/02/06/my-…
'Dad was a hard-working air conditioning engineer and he loved his job but his long hours began to catch up with him.'

'For a better work-life balance, he became self-employed.'
'This transition led to bouts of unemployment and a squeezed income.'

'He struggled to keep up with the paperwork; the people he worked for grew increasingly concerned about his performance.'

'Little did I know these were the early stages of dementia.'

1/🧵Important = Brain Injury in #COVID💥

📍N=251 hospitalized COVID pts had biomarkers of neurodegeneration (dying brain cells).

📍Previously healthy COVID pts had ⬆️er levels than Alzheimer’s pts!

How do we understand this?
Hint: #Vaccinate yourself

bit.ly/3frTcbB
2/ These elevated brain biomarkers indicate disease to different types of brain cells (eg, astrocytes, glial cells, neurons) in COVID patients. These were early unvaccinated pandemic patients. Vaccinated people will be less prone. WHY?
3/ There isn’t a ton of brain invasion by COVID. We think most of the 🧠 injury is INDIRECT. Meaning downstream injury from 1. upstream inflammation 2. blood clotting & 3. heavy sedation causing #delirium, which we proved to be a #dementia risk factor!

1/🧵 Brain + Body damage in #COVID & Critical Illness

Sepsis -bacterial, viral, fungal- can cause rapid-onset #dementia. Finishing this test should be easy at her age but Millions of brain cells are damaged!

Protect yourself & others: #Vaxx + #Mask

H/T @trishla_17 for helping
2/ Our brain depends on a delicate balance of blood flow, nutrients & oxygen. When someone gets critically ill w #Covid or from other causes, that balance is dramatically disrupted. Small blood clots, dropping oxygen levels cause 🧠 damage.

bit.ly/36K5F65
3/ It’s odd to me that people don’t see severe COVID as a form of VIRAL SEPSIS. Sepsis is when infection of any kind causes your organs to begin to fail – lungs, brain, heart, kidneys.  It’s EXACTLY what we see day-in day-out in the COVID ICU.

Lifestyle habits which can be bad for your brain health.
#Dementia
timesofindia.indiatimes.com/life-style/hea…
While the condition is primarily caused by physical changes to the brain matter, heavily dependent on one's age, genetic risk, family history or if injuries to the brain, the kind of lifestyle you lead, dietary deficiencies may also increase one's chances of developing dementia.
If there is one single factor which can increase your chances of cognitive decline, poor brain health and accelerate ageing, it has to be the lack of Vitamin B12 in our diets.

1/🧵For months I’ve been grappling with what seemed like a real gap in conversations about #LongCOVID.

Here in @STATnews, I introduce you to 30 references & 3 #COVID survivors – Ray, Pam, and Carolyn – who taught me a lot. I hope you think so, too…

2/ "I feel like I'm getting the silent treatment & it's killing me," #LongCOVID pt Pamela Bishop confided in me about her months-long interactions as she tried to get answers about a strange array of symptoms that have plagued her since recovering from #Covid19.

Pam then & now…
3/ Up to1 in 3 COVID survivors report experiencing #longCovid symptoms 3 to 6 months later. Their stories give me an extreme case of déjà vu because this is playing out similarly to the problem of long-term survivorship after non-COVID critical illness.

People with #dementia for example may be unable to check in this way due to having difficulty learning new things.

1/🧵Wake-Up to how #SLEEP flushes waste from your brain 🧠 @JAMA_current

I may get more done when I 💤 less, but I lose the long-game! #Evolution dictated that we gain an advantage by spending 1/3rd of our lives asleep.

I’ll tell you WHY? bit.ly/3qPgzAA

#MedTwitter
2/ Why do we sleep? Brain rest? To process bad decisions or incomplete conversations? Maybe. But the coolest reason is removal of #amyloid & #tau proteins that cause #dementia. Our #glymphatic system does this most efficiently during deep sleep.

3/ Have I mentioned that I’m fascinated by the glymphatic system? It’s the lymph system for our #brain & serves as the “garbage” removal program to keep our #neurons healthy. Wanna know more?

Today, our family followed through on the decision to move dad to a long-term facility in Mississauga. He's been on a dementia journey for about 8 years alongside a cancer one that has him also receiving palliative care. /2
The script flipped for me was a few years back when his conscientious nature had him scaling an aluminum ladder in a rainstorm to check that the eaves were clear ensuring we wouldn't have any flooding. Both his physical and mental capacity at the time put him at risk. /3

1/ 🎥🧵 How do we test someone for #delirium in #COVID19 or any ICU? When someone won’t engage it can be a huge red-flag for “quiet” delirium, which predicts 3X ⬆️ #DEATH by 6 mo & acquired #dementia. Watch this movie of me testing pt #1 for delirium.

#MedTwitter #NurseTwitter
2/ What does this mean? He starts out OK but then can’t follow my command to squeeze only on the letter A. Instead, he squeezes when I say the word “squeeze” & doesn’t relax his grip or vary it on different letters. This is inattention & a cardinal feature of #Delirium.
3/ 🎥 Contrast Pt 1 with this Pt 2 video of me doing the same test. This person had delirium on earlier days & is now improving. He gets 8/10 correct responses, which means he’s attentive and not delirious any more. This day he walked & talked for the 1st time!
