According to the New York Times, both Paul Manafort and Rick Gates provided polling data—including some prepared privately for the campaign—to Konstantin Kilimnik, with instructions to pass it along to Oleg Deripaska.

UPDATE: The New York Times has issued a correction stating that Manafort asked Kilimnik to pass the information to the Ukrainian politician Serhiy Lyovochkin and oligrach Rinat Akhmetov, not Deripaska.
Both have played significant roles in the Russia scandal already: Akhmetov, a ruthless mining magnate with ties to organized crime, initially brought Manafort to Ukraine in the 2000s and funded his work for pro-Kremlin politicians there ... themoscowproject.org/explainers/eve…

Improper redaction by Manafort's legal team reveals that Mueller has alleged that Manafort shared polling data related to the 2016 election with Konstantin Kilimnik, then lied about it to investigators

The text, which can be read by copying the text underneath redactions on page 6, claims Manafort didn't lie about "sharing polling data with Mr. Kilimnik related to the 2016 presidential campaign" but merely did not remember it during proffer sessions.
Trump and Kilimnik, whom Mueller's team has alleged had ongoing ties to Russian intelligence during the election, also allegedly met in Madrid, although the documents do not specify when the alleged meeting took place.

According to @kenvogel, the first to post the document, KC refers to Kypros Chrysostomides, a Cypriot politician who also helped facilitate Manafort's under-the-table payments from pro-Kremlin Ukrainians:
Cyprus has historically been a favorite tax haven for Russian money launderers—often through a bank that once counted Commerce Secretary Wilbur Ross as a board member, and the Russian oligarch Dmitry Rybolovlev one of its biggest shareholders.

What are the key takeaways from the #Flynn sentencing memo and “addendum”?

Two, Flynn assisted a criminal investigation that is completely redacted, so presumably not a known criminal investigation. There may be something else going on...
Three, “investigations in which he provided assistance are ongoing.”
Maybe the end is not so in sight as some suggest.

What we know:

Flynn was cooperating with at least two investigations—at least one of which was a criminal investigaton—outside the Special Counsel’s office.
1/ This means the Mueller probe is fairly safe. Why?

Because the evidence is spread around to various offices. The redacted portions of the addendum can be made public at any time, hence informing the public about key details.

Also it's clear to me Mueller has the whole story.
2/ What other criminal investigation did Flynn assist with? We can only guess.

We also know that Flynn started cooperating early, and he provided substantial assistance to Mueller, and his timely cooperation persuaded “related first hand witnesses” to co-operate.

No, that's C. Because the first set of redacted subheads is B(ii). That subhead is redacted entirely.

NEW: Mueller's office says former National Security Adviser Mike Flynn "has assisted with several ongoing investigations, including its probe of coordination between the Trump campaign and the Russian government.
Mueller's office says that it can't reveal the details of all of Flynn's assistance because "the investigations in which he has provided assistance are ongoing."
Among the matters on which Flynn has provided assistance to the government is a criminal investigation - apparently separate from the collusion probe - that remains totally secret here.
