For the ACA boosters in the back of the room -- @NeeraTanden, you! -- this sort of thing is exactly why it's absurd to suggest the ACA is some form of "universal health care." Universal health care systems don't allow patients to be financial victims of contract negotiations.

It's borderline monstrous to name a policy the "Affordable Care Act," which allows parasitic actors like emergency room doctors who refuse to accept negotiated insurance company rates, to balance bill patients for thousands and thousands of dollars.
The ACA did *nothing* to ban balance billing and surprise bills. Nothing.
