A common habit resulted in severe #vision impairment in a young woman
1. 30-year old Manju had severe disabling vision symptoms for one and half years. This included seeing floaters, bright flashes of light, dark zig zag lines and at times inability to see or focus on objects.
2. There were moments when she could not see anything for several seconds. This occurred mostly at nights when she got up to use washroom. She was evaluated by an #eye specialist and detailed evaluation was found to be normal.
She was referred to rule out neurological causes.
3. I reviewed the history. Symptoms had started after she quit her job of a beautician in order to take care of her specially abled child.
She picked up a new habit of browsing through her smartphone for several hours daily, including >2 hours at nights with lights switched off.
4. The diagnosis was obvious now. She was suffering from smartphone vision syndrome (SVS). Long-term use of devices such as computer, smartphones or tablets can cause various eye-related disabling symptoms, referred to as computer vision syndrome (CVS) or digital vision syndrome.
5. I did not order any investigations nor did I prescribe any medicines (even though Manju requested, as she was anxious). I counseled her about the possible cause for her vision impairment and suggested her to minimize the use of smartphone. #MedTwitter#NeuroTwitter
6. Manju was anxious as she feared something sinister with her brain nerves but was finally determined to take corrective action.
She said- "instead of minimizing, I will stop looking at smartphone screen, unless absolutely necessary. In any case, my phone use is recreational."
7. At 1-month review, Manju was absolutely fine. Her vision impairment of 18 months had gone. Now, she had normal eyesight, did not see any floaters or flashes of light. Moreover, her momentary loss of vision at nights also stopped.
Our suspicion was proved right.
8. Take home message:
*Avoid looking at screens of digital devices for long, as it can cause severe and disabling vision-related problems.
*Take 20-second break, every 20 min, to look at something 20 feet away, while using a digital screen (20-20-20 rule).

Being female, improper body posturing, use of electronics devices out of work, no habit of taking a break, long-hour duration of visual display terminal use & short-distance screen were associated with increased odds of CVS
11a. Spectral peaks for smartphones are very similar to short-wavelength blue visible light. Damage to retina caused by light, especially blue light is well-established.
A case of retina damage causing blurring of vision in both eyes due to smartphone use at nights was reported.
Vitamin D is effective in decreasing risk for diabetes in adults with prediabetes
1. In a meta-analysis, the risk of #diabetes reduced by 15% in adults with prediabetes, who were given #vitaminD: 20,000 IU (500 mcg) weekly; OR 4,000 IU (100 mcg) daily, as compared to placebo.
2. The 3-year absolute risk reduction in developing diabetes was 3.3%.

3. There are certain limitations.
*The effects of vitamin D can not be generalized to healthy adults, as it was tested in only those with prediabetes.
*The safety of prolonged vitamin D use on regular basis is not yet fully clear.
1. Running has several benefits including lesser risk of death and longer #survival. As per a research study, persistent running over 15 years can add 5.9 years to life. doi.org/10.1016/j.jacc…
2. As per study, runners added 3 years to life as compared to never runners, and persistent runners (who continued running through the follow up period) added 5.9 years to life.
All cause death reduced by 29% and cardiovascular death reduced by 50% among consistent runners.
3. The benefits on survival and reduction in death were seen in all runners.
Weekly running even <51 min, <6 miles, 1 to 2 times or <6 miles/hour was sufficient to reduce risk of death, compared with not running.
Relatively higher risk of Guillain-Barre Syndrome (GBS) after Janssen #COVID19#Vaccine

3. Take home message
*Adults should be screened for the presence of depressive symptoms and those with depression should be treated, in order to reduce the risk of cognitive decline and accelerated brain aging in later life.
An elderly man who repeatedly kicked the woman ahead of him while waiting in queue to board a flight
1. 69-year old Balu, while waiting to board a flight, started to repeatedly fling his right leg that hit the woman standing in front. The woman was shocked with this incident.
2. She was terribly upset and remarked-"just because boarding is delayed & you are restless, doesn't mean you can kick me like that. Moreover, you can't take advantage of you being older to me".
But Balu denied any wrongdoing and said the leg started shaking without his control.
3. As soon as Balu landed in Hyderabad, he consulted a physician, who diagnosed focal motor #seizures (fits) and started him on anti-epileptic drugs (AEDs). Similar symptoms repeated after one month despite being on regular AEDs, and he was referred for my opinion. #Medtwitter
