An elderly man who repeatedly kicked the woman ahead of him while waiting in queue to board a flight

1. 69-year old Balu, while waiting to board a flight, started to repeatedly fling his right leg that hit the woman standing in front. The woman was shocked with this incident.
2. She was terribly upset and remarked-"just because boarding is delayed & you are restless, doesn't mean you can kick me like that. Moreover, you can't take advantage of you being older to me".
But Balu denied any wrongdoing and said the leg started shaking without his control.
3. As soon as Balu landed in Hyderabad, he consulted a physician, who diagnosed focal motor #seizures (fits) and started him on anti-epileptic drugs (AEDs). Similar symptoms repeated after one month despite being on regular AEDs, and he was referred for my opinion.

Uncontrolled #epileptic fits stopped by #insulin

1. A man aged 75 had for five days experienced tonic-clonic #seizures involving the right side of the face and right upper and lower limbs at a frequency of 20-30 per day. He had mild weakness of right arm and leg.

2. Patient was admitted in ICU under Medicine Unit. MRI brain and lumbar puncture were normal. New onset diabetes was detected for which he was treated with subcutaenous insulin.
Fits persisted despite treatment with midazolam and phenytoin. He was referred for neurology opinion
3. Patient was conscious but appeared exhausted. Review of blood tests showed high glucose of 350 mg/dL and mildly low sodium levels.
The diagnosis was obvious by now.

The book is more clinically oriented, coloured with anecdotes and mental models. Reading it feels like seeing a pt in the ward/OPD

Bradley is the standard #textbook in #neurology. A great book, it’s more like #Harrison. Great for information, latest research and management. Essential for the DM #neurology candidate, but also useful for MD #internalmedicine

A man who would become unconscious after singing romantic Kishore Kumar's Songs
1. 6 months ago, a 45-year-old man visited me for evaluation of a fainting episode that occurred while he was singing for a private gathering. He was a schoolteacher and singing was his passion.
2. On the fateful day, he had uneventfully sung a few songs of other singers.
On public demand, he sang “Pal pal dil ke paas”, a popular Kishore Kumar's song. A few minutes after completing the song, he collapsed on the stage and became unconscious.
3. His pulse rate was 110 beats per minute and BP was 160/100 mmHg. He was shifted to a hospital for evaluation.
He had regained consciousness while being taken to the hospital.
ECG, Echocardiogram and Cardiologist evaluation were normal.

Cause of coma in a #woman surprised us!

1. 65-year old woman was brought in an #unconscious state to #emergency dept. She sounded normal over phone to her son about 24 hours earlier. She stayed alone, so, further details were not available.

2. Clinical examination revealed an #unconscious patient. She was afebrile; pulse and BP were normal. Pupils were equal and reacting to light. Meningeal signs were absent. She was electively mechanically ventilated in view of her unconscious state.
3. Serum #calcium was high (13 mg/dL). MRI brain was normal. Lumbar puncture was normal. Investigations were done to determine the cause of high calcium. There was no evidence of #endocrine disorder, infection or #Cancer.
#VitaminD level was very high (160 ng/mL).

1. Water is life-saving and is essential for survival. However, ingestion of excess amounts of water, especially over a short period of time can lead to serious health hazards, including #death.

2. How much water is needed on daily basis?

The normal daily requirement of water is 2.5 to 3 liters.
The requirement may be higher in summer months. Requirement also increases during #exercise (due to sweating), during fever, diarrhea or vomiting.
3. Why do some people drink excess amounts of water?

There is a misconception that drinking more water is good for #health.
Some people can drink more water due to #psychiatric illnesses.
#athletes can overestimate fluid loss and drink an excess amount of water.

1. Peripheral #neuropathy can be the first manifestation of diabetes. 50-70% of diabetics suffer from neuropathy. Pain and burning sensation of feet are common symptoms. Numbness of feet can predispose to ulcer formation.

2. Diabetes is a common risk factor of #stroke, seen in 30% of stroke cases. Diabetes leads to poorer outcome in stroke patients- longer length of hospital stay, higher risk of complications, higher risk of death due to stroke and greater disability in stroke survivors.
3. Diabetes is associated with higher risk of dementia including Alzheimer's disease. The risk of mild cognitive impairment (MCI) and conversion of MCI to dementia is higher in diabetics. Longer duration of diabetes and uncontrolled diabetes further increase the risk of dementia.

Oh my goodness, what I am learning about the lived experience of temporal lobe epilepsy is mind-blowing! People can be misdiagnosed with psychological whatnots for years, and be gaslighted into oblivion because of the arrogant ignorance of psych professionals! This is terrible!

Thread, a reflection:
It's hard to describe how often throughout the work day I'll surprise myself by how 'on it' I am.
Before #keto, while on massive doses of #epilepsy meds, I would sit in a meeting, take exhaustive notes, yet struggle to recall action items a mere hour later.
I would have multiple projects running simultaneously and struggle with remembering which deliverables were for what campaign, the resources assigned - people would ask me questions and I would have to dig through pages of notes to come up with the answer.
I regularly suffered from impostor syndrome. I didn't feel competent in my work, especially when running meetings. My colleagues had it so together and I felt like such a mess, unable to recall even the most high-level details.

3/ as the pandemic affected Europe and North America ahead of Latin American countries, producers produced and traffickers tried to traffic as much as they could to build #cocaine reserves. This is why we had huge seizures in April/June last year. So that they wouldn’t run out.
