Human Error is not that human after all. 
When we talk about human error, we immediately think it's about the person. In reality, human error is not just about the individual; it's about human behavior.
The problem with human behavior is that it is not that simple to understand, let alone explain.

Sadly, little is known about human performance failures and their causes. We tend to blame the human, ignoring the fact that humans have limitations and that we operate based on systems, procedures, and processes designed by the organization.

We decided to learn from them. Why? Because we wanted to fix the problem not the blame. We needed results, and we needed them ASAP. That's the beginning of a fantastic journey to find the answers.
Today we know. Today we can do something about it because it always was about discovering the truth. We can proudly say that we have been 100% successful in reducing human related events.
We take the time to understand how, when, where, how, and why we do what we do the way we do it.
We establish the type of error whether it's an intentional action (no harm intended) or if it was an unintended action clearly out of our control. Nobody wants to forget to do something important.
