... on receiving a paper like this, journals should have worked to publish it rather than fade it for lack of "novelty";

1 use it when talking to people about going on to renal replacement acutely
2 ask how this can be? How can half of those we start on RRT be dead before three years are up? How do we do the best thing, knowing this?

I think the answer is not to start on the interpretation hamster wheel, to blame the victims or talk within our own bubble, but to ask those affected and their mob, and listen to the answer:
