He wished to go to Russia for his treatment but Russians also refused for they didn't want to be blamed for the death of a Pashtun leader.
Gen. Zia offered him to come to Pakistan for treatment or fly with him in his plane to Russia for treatment but Bacha Khan refused.

Bacha Khan didn't had Pakistani passport, but for international travel he needed one. So he applied for Pakistani passport which was issued to him after some difficulties. He would later go to India for his treatment.
Source of the above is a Bacha Khan interview from 1980. He was interviewed by Haroon Siddique an ndian journalist for the Toronto Star in Canada.
It is the same interview where Bacha Khan confessed that the idea of Pakhtnistan was exploited by India and Afghanistan for their political agendas.
