Food myth busted: Potato is not bad for diabetics

1. Potato has got a bad name, and diabetics are advised to avoid it because of its high glycemic index. However, findings of a recent research busts this myth.

2. This was an 8-week randomized, controlled feeding study comparing Potato and Bean diets (50-55% carbohydrate, 30-35% fat, 15-20% protein).
Thirty-six participants (18-60 years, BMI: 25-40 kg/m2) with insulin resistance were enrolled.

3. Among study completers, HOMA-Insulin resistance in the bean group showed an average decrease of 1.4 from baseline (P = .02 ); a similar decrease of 1.3 occurred in the potato group (P < .05) with no significant difference between the two diets.
4. Body #Weight reductions were similar in both groups with average reductions of 5.82 kg in the potato group and 4.0 kg in the bean group.
#BMI also was significantly reduced from baseline in both potato and bean groups (2.04 kg/m2 and 1.35 kg/m2, respectively).
5. This study dispels the myth that a small portion of potato in the diet (although these are not potatoes that are fried, or are topped with cheese, bacon, sour cream) results in deleterious metabolic outcomes when compared to a diet that is comprised of beans (pulses)-Medscape

A man who would become unconscious after singing romantic Kishore Kumar's Songs
1. 6 months ago, a 45-year-old man visited me for evaluation of a fainting episode that occurred while he was singing for a private gathering. He was a schoolteacher and singing was his passion.
2. On the fateful day, he had uneventfully sung a few songs of other singers.
On public demand, he sang “Pal pal dil ke paas”, a popular Kishore Kumar's song. A few minutes after completing the song, he collapsed on the stage and became unconscious.
3. His pulse rate was 110 beats per minute and BP was 160/100 mmHg. He was shifted to a hospital for evaluation.
He had regained consciousness while being taken to the hospital.
ECG, Echocardiogram and Cardiologist evaluation were normal.

Giving #Paracetamol in cases of mild #Viral #fever may be harmful
1. It is a common practice to give paracetamol (acetaminophen) to children or adults suffering from viral fever. Research evidence shows that we may be causing more harm than good by this practice.

2. Fever is not a disease; it is a symptom of viral infection. Paracetamol can not kill the virus.
Acute infections, including those due to Corona virus and other viruses, often stimulate a febrile response.
3. A mild fever appears to improve #outcome; as it diminishes viral replication by several mechanisms, including virion entry into host cells and genome transcription, and improving host defense mechanisms against the pathogen.

Painful muscle cramping in runners- why do they occur and how to manage them?
1.Many runners have experienced cramps (involuntary painful muscle contractions). Besides pain, it can derail one’s aim for the personal best; in worst cases, runner is unable to even complete the run.

Shorter #stride length is associated with higher incidence of life threatening clinical events in #olderadults
1. 14,000 people (Av age 75 yrs) were followed up for 33 months. 10% of them sustained #Falls. Stride length was shorter in fallers as compared to non-fallers. Image
2. In a sub-analysis based on gender, the stride length was longer and #walking speed slower in males compared to females.

"Retirement" from work as a risk factor for #Dementia: experience of 5 recent patients

1. I saw 5 new cases of dementia last month, which apparently started within 6 to 18 months after retirement from work. All were cognitively normal until retirement.

2. Memory impairment was the most prominent symptom. Over a period of time, other symptoms such as impaired calculation, speech impairment, losing sense of direction, poor judgement and inability to do daily activities without assistance, also started. Symptoms were progressive.
3. A diagnosis of Alzheimer's disease was made on the basis of detailed neuro-psychological assessment, brain scans and other appropriate investigations.
Two patients had comorbid #depression. Three patients had #sleep disorders- two had #insomnia and one had excessive sleep.
