1/ Much appreciated. Based on the work designed by Dr. Scott Zeller's team. I had an audiogenic seizure + middle ear damage followed by a fall and rotator cuff injury in 2017. I didn't go to the ER.

2/ I was highly photophobic, dysosmic and in acute hyperacusis. With the middle ear damage, I couldn't walk on a straight line. The ER would be a risk. I stayed home, took whatever drugs I had and half a bottle of Bourbon.

3/ Because of that, the recovery was never complete. The pain and neurological disorder got worse, the first physician to see me made several mistakes and the cycle of damage continued until an almost lethal outcome.
4/ This could have had a totally different outcome if I had proper emergency care. It's time to implement that.

1/ Although I still don't think the probability of a guerrilla-level uprising will happen during the upcoming clash between the rising global far-right and what is loosely known as "democratic forces", it doesn't mean there won't be violence.
2/ The best case scenario (yes: there is nothing better than this) is the "democratic fronts" win over the far right and push them back into marginal corners of society and economy. Their leaders will still control a good part of the "secondary economy" (criminal organizations).
3/ The low-level RWA, the *mass* that sustains their populism, will be extremely upset and angry. There are few things that will exert sufficient persuasion over them so as to (metaphorically) "push them back to the hills". It will be coercive (best case scenario).

1/ It’s been a week.

It’s well into Friday evening & I just got off the phone with a patient who had urgent radiology results. This was after spending > 40 min on the phone w another patient discussing the same this afternoon.

(18 more to come; stick with me.)
2/ I only got 2 items on today’s long “to do” list done — but that’s on me. I was finally able to sit down for a couple hours uninterrupted and dig deep into a few patient cases from the week.

(And I “don’t work” on Fridays.)
3/ As I hung up the phone, I reflected on how #directprimarycare has enabled me to get to the heart of medicine & the doctor-patient relationship.

During that call, my patient’s spouse broke down and — through sobs — simply said: “Thanks for being so kind.”

I am so sorry. We want our children to be safe, but part of being safe is realizing that not everyone is an ally. Not everyone sees you; they see your skin, or the gender you present, or the friends surrounding you. People label, people make assumptions.
People who give themselves a rotator cuff injury patting themselves on the back over how accepting they are say, and do, stupid shit.

What to do with this information?

Before Disney got his hands on them, fairy tales used to be dark.

1. "#Happiness" can scare #anxiety patients. In our heads, things are always going wrong. Fear of a "happy" moment undone is doubly painful.
