So hey, if you roll up to tell me that you are magically the Fat Whisperer, and can tell at a glance whether someone needs your health advice, because they wear big pants or need a seat belt extender, or whatever, I will block you, and you ARE a bully.
Times your health advice is desirable:

1. You are my doctor.
2. I am actively gushing blood from multiple non-vaginal orifices.
3. I have asked for it, using my words, not with "body language" or "a silent cry for help" or some such bullshit.
My brother is thin enough that a stiff wind can move him, and he's generally less healthy than I am, even though people make Jack Sprat jokes when they see us together. You don't know. YOU DON'T KNOW.
But if you can never know peace without saying "hey you're fat and that means you're UNHEALTHY, EMBRACE MY WISDOM," then no peace for you, you fucking bully. Do you also accost everyone you see eating a cheeseburger, or drinking a beer, or texting and walking at the same time?
If this hurts your feelings, if you don't like being called a bully, there's an easy fix. STOP FUCKING TALKING TO STRANGERS ABOUT THEIR HEALTH WHEN THEY HAVEN'T ASKED FOR YOUR OPINION. That's all! Full stop! It's so easy not to be a bully!
And yeah, sometimes, with friends and family, there are appropriate questions about weight. "I noticed you lost a lot of weight but you've been eating normally, is everything okay?" can potentially be safe, with a close friend.
But that's still dangerous, and I urge you to think twice before bringing up weight with someone who didn't bring it up first.
