vaping-induced lung injury cases continue to rise (now 450 cases & 5 deaths). this thread summarizes some key points from fresh articles in NEJM & MMWR to help us stay on our toes with this emerging disease (1/8).
epidemiology:
- mostly young men (likely reflects the population using modified vape products)
- most patients report vaping with tetrahydrocannabinol (but not all)
- generally previously healthy (2/8)

upon admission, most patients have symptoms in three domains:
1) respiratory (dyspnea, chest pain, cough)
2) GI (n/v, diarrhea, pain) - may be *dominant* symptom initially
3) constitutional (fever, chill, weight loss, fatigue)

Upper respiratory tract usually uninvolved (3/8)

most important point = don't stop buprenorphine due to acute pain! dividing the bupe dose q6-q8 may improve analgesic effectiveness, so buprenorphine itself can be used as an analgesic. (2/5)
you don't need an X-waiver to prescribe buprenorphine within the hospital. using buprenorphine is now a core clinical competency that all inpatient providers should be comfortable with (3/5)

Stepping back from the culture wars for a short thread on #Japan & #Religion. Believe it or not, this is my specialist area of research and as I finish up a new paper on the topic, I thought I would share some interesting results. Strap in!
The popular image of Japan & religion is something of a paradox. On the one hand, you have lots of pieces about Japan as a country of non-believers. This is usually based on survey data where only 10-15% of Japanese people self-identify as religious.
Indeed, we saw this exact pattern in data we collected from 1,000 Japanese respondents in 2016. When we asked whether they would describe themselves as religious, regardless of participation in religious events, we got the following responses: only 10% identified as religious.

So. I’ve been all over the world looking for ghosts. I stay at haunted hotels and do ghost hunts and lurk thru cemeteries at night. In all that time I’ve only seen one real ghost. This is that story.
Twenty years ago I went to the attic of an antique store in San Angelo (Texas) where I lived. In the attic was a horse drawn wooden hearse carriage. The whole attic felt oppressive. There was something there.

The images below are of the recently tarred service lane at Oshodi, work here was completed barely 6 months ago.

So, apart from the obvious poor quality of work done, why & how exactly did the Potholes occur?

The Wikipedia post basically outlines the issues as a failure in the fabric of the road due to moisture beneath the road surface & fatigue from vehicle load.

The article points more at snow as being the moisture enabling culprit but we obviously don't have snow in these parts.
'Snow' may therefore be substituted for another firm of precipitation - rain (and subsequently water).
