A fresh discussion on an old topic
Like every problems in this world, the factors involved are intrinsic and extrinsic
Intrinsic being the doctors and the system
Extrinsic being the patients and society

1. Doctors
Generally doctors are efficient.Generally. But because of maldistribution city/rural or doctors pulling strings (parents' supports to be in specific hosp) or stunts (unplanned EL/MC,regular cig breaks), the services at certain clinics/hospitals will be affected

2. System
Patientload getting more because of poor health education/campaign. Not uncommon to have diabetic awareness talks/program comes with heavily-carbs nasi lemak. Even hosp staffs smoke within hosp vicinity. No audit on success of public health programs

Although the Ministry is against staff abuses, but when it happens, the staffs themselves can take legal action against the abuser but the Ministry will be "spiritually supportive". A handful of top Pengarahs are champions in replying complaint letters even the nonsensical ones

Generally the system allows those who complaint thru socmed or physically to get a better services which positively encourages or even aggravates such action in the future

3. Patients
As for the patients, well, I had a seen a fair share of grump old ladies and men(their partner) and sometimes failure to see bigger picture made them angry. As for youngsters in fast-paced tech world, they want instant gratification

4. Society
Society in general does not appreciate cheap stuffs. Social experiments aplenty. World famous violinist Joshua Bell plays in MRT - nobody bats an eyes, his expensive music show faces loud applauses

So, what can the doctors do in such a situation?
Or rather what did I do when I faced with such a situation...
OK story time...

In a hospital, usually for emergency admission, usually there is two entry point, one is through the emergency (A&E) department and the other point is the patient admission centre (PAC) specifically for pregnant ladies which is near or semi-attached to labor ward.

A&E and PAC can sometimes be far apart. In this particular hospital, they are separated by a wide 4-lanes road
There was an ambulance call
A pregnant mom 24weeker fell in toilet to the back and she claimed she can't move her 2 legs.
The general rule is preggy goes to PAC

Thus, this unfortunate lady was sent to PAC,I was there in PAC and because the patient can be pushed down,I asked the ambulance to sent to A&E to enable earlier orthopaedic assessment
Her husband wrongly accused me of rejecting her wife and gave me bengis look

Then,unexpectedly,he whipped out his nametag and shouted "I am XSY, reporter with Media XX,I will report you and make you famous!"
I didn't wanna miss the chance to be famous as well so I politely tell him....

I took out my work identity card &wrote my name/ic/hp no on small paper
"Here's my ID with photo & my hp no,if u reli nd someone to talk or wanna write bout me"

In another separate hosp or incident,mom of high profile boss of media company was admitted.

I had to explain to him in detail about his mom and he actually recorded a full video of me doing the counselling

I didnt stop him halfway bcos I thought of telling him to delete later

But at the end,he said politely "I just wanna show to my wife that not here tonite and I will delete it at home. Trust me ya, bro."

Awkward silence 1 min followed by "Ok,I guess"
