My father is a doctor & became a doctor myself. I graduated in KPK, served in KPK for a year & then came to UK & trained to be a consultant in NHS; while being in contact with the implementation of MTI in KPK. This is my opinion on MTI act.
In present health system, individual hospitals have to get most of the major decisions verified from the central health department leading to inertia and stasis. In MTIs individual institutes can make decisions locally leading to improved efficiency.
Presently, majority of recruitment is done centrally from the health department. If a hospital needs a specialist, post has to be created by health department before recruitment takes place. The delays are sometimes decades. MTIs can recruit to their own needs.
Presently, doctors employed in Jhelum can get transferred to Lahore on sifarish leading to deficiency of doctors in peripheries & congestion in centres. In MTIs, employees will have to work in the hospital they have been recruited in.
Presently, disciplining an employee is extremely difficult as the decisions are made centrally. In MTIs, individual hospitals will have local procedures to hold doctors accountability. As we know, there can never be success without accountability.
In big central hospitals there are dozens of trainee doctors in each ward. Sometimes, one doctor for a single patients. In MTIs, hospitals will only induct numbers that can be trained well. Trainees will be equally distributed throughout the whole province.
In MTIs, the employees will have to do their private practice in hospital. Presently, doctors earn mainly in private setups while working in Govt set up. This leads to conflict of interest & is the major reason why Govt hospitals do not improve.
Through hospital based practice, MTIs will generate revenue to then invest in poor patients.
Private health care is a multi billion rupees industry & even if MTIs get 5% of the share, poor patients can benefit a lot.
For example, those doctors who have well established private practice or outside laboratories might not want the hospital to provide the same service that will be his/her competition.
