2/11 #StateOfCare identifies the challenges faced by people with multiple needs and describes people and their families needing to chase services to get the right care. @RichmondGroup14 work on multiple conditions has also found people feeling exhausted and overwhelmed. 3/11 #StateOfCare shares a story from @RichmondGroup14 member @age_uk that shows how people can sometimes be viewed by professionals with a narrow focus, especially outside obvious moments of transition. Page 19 of CQC report.
4/11 #StateOfCare highlights gaps in community services and wider community sector support, such as befriending schemes. Big ambitions in #NHSLongTermPlan on these issues but will need sustained focus and an understanding of how council cuts have hollowed out local capacity. Page 20 of CQC report.

9/11 Strong focus on #innovation in #StateOfCare. Good to see @CareQualityComm highlighting people’s experiences and putting service user needs at heart of innovative design as key factors in successful service innovation.
10/11 Important acknowledgment in #StateOfCare of role played by voluntary sector in supporting people, families and friends as they encounter gaps between services. Page 54 of CQC report.
11/11 #StateOfCare also drawing upon insight from @RichmondGroup14 member @Rethink_ to show how technology has potential to reduce inequality and promote people’s rights. Page 90 of CQC report.
