*New* @TheLancet : We suggest that patients w/ cardiac diseases, hypertension, or diabetes, who are treated with ACE2-increasing drugs, at higher risk for severe #COVIDー19 infection & therefore, should be monitored for ACE2-modulating medications, such as ACE inhibitors or ARBs
The most distinctive comorbidities of 32 non-survivors from a group of 52 ICU patients w/ #COVIDー19 were cerebrovascular diseases (22%) & diabetes (22%).

Another study 173 had severe disease w/comorbidities of htn (23·7%), diabetes mellitus (16·2%), coronary heart diseases (5·8%), & cerebrovascular disease (2·3%). Third study of 140 patients who were admitted to hospital with COVID-19, 30% had hypertension & 12% had diabetes.

The city of Wuhan, China, where #COVIDー19 started, waited weeks before acknowledging human-to-human transmission and taking measures to control it. Wuhan thus experienced an out-of-control epidemic that overwhelmed the health care system.
Other cities in China watched Wuhan’s experience and imposed strict controls at a much earlier stage in their epidemic: They closed schools, sharply limited social contact, and traced and isolated cases and contacts. These early interventions dramatically slowed transmission.
These experiences, & historical parallels, like 1918 flu, show early and sustained imposition of measures to limit social contact will slow the epidemic. This is desirable for many reasons — fewer total people get infected in a slowly moving epidemic... bostonglobe.com/2020/03/11/opi…

#Coronavirus has been compared to influenza. But there are no drugs or vaccines yet for treating or preventing the virus as there are for flu. We are exposed to different flu strains which help us build immunity. No one had immunity to this virus.

Most descriptions of children with #COVID-19 have been reassuring, few have had severe disease and most recover rapidly. However, there is some concern that immunocompromised children will fare much worse. (2/12)
This case in the Chinese Journal of Hematology is reportedly the first case of documented COVID-19 infection in an immunocompromised child. It’s in Chinese and I had a lab colleague read it to me – apologies for any errors. (3/12)

As the US and many other countries brace for an uptick in new infections, hospitalizations, and deaths due to #COVID-19, here is one other problem almost no one is talking about: long-term chronic illness.
It can often be triggered by a severe viral infection, could be a far more common outcome than death, and yes, afflicts the young.

A challenge w/ #COVID is that panic, itself, is contagious—an “infodemic.”

Without strong, evidence-driven leadership, local leaders default to the most stringent policies.

We’re seeing this at all levels—from banning travel from Europe to shutting down schools.

For example, School closures MAY be right, but we don’t have any evidence; & they could do harm:

1) healthcare workers who are needed on the front lines have to scramble for care

2) it forces seniors—at most risk—to care for kids.

3) tons of kids rely on school lunches.

AND we know that kids actually have low risk of serious #COVID. Only 2.4% of all cases in one study.

Rather than make policy specific to #COVID, ppl are borrowing policy from other infectious epidemics, like flu, which is super deadly for kids.

As stated before:

Finally authorities are putting Mazandaran province on quarantine: only fuel and food trucks are allowed. This late, its effectiveness will be limited.

The same authorities have had argued for quarantine to be "non-scientific" and "from middle ages".
