Thinking a bit more about a hypothetical second vote or people's referendum:
A) Regardless of what options are on offer, there shouldn't be any that introduce new uncertainties, so no "Negotiate a better deal with the EU, then leave" type option. "Deal" is WA+PD, as they stand.

B) The case for having "Remain" on the ballot becomes much less contentious if "no deal" is also on the ballot, because the hard Brexiters aren't shut out
C) But "no deal" MUST be more clearly worded, e.g. "Leave the EU without any deal."

a 3-option ballot seems the most inclusive. Harder to run, harder to explain, but doesn't leave any faction steaming on the outside of the process because their preferred outcome was disregarded entirely.
Taking all of that together leaves us with something like:

Two columns. "1st preference" and "2nd preference". Pop a cross in each.
Not perfect. Not even particularly great. But taking the country as a whole, may provoke the least outrage.
