Little thread about #trans healthcare in the UK.

1) I was not “permitted” top surgery, because I needed to “live as a man” first. According to the #NHS. The private psychiatrist who diagnosed and referred me, and the private surgeon who did the op, had no such reservations.
I paid over £7,000 that I couldn’t afford to relieve the worst of my suffering. Because the #NHS knew better than me.

2) Then I needed #HRT. My #NHS GP refused to get involved and tried to put me on a four year waiting list for...a diagnosis. Which I already had.
Ironically, when I called Leeds GIC, they agreed it was a waste of time and gave me the details of the endocrinologist they typically use. It still took two months to persuade my #NHS GP to make the referral.

Trans kids have ridiculously high rates of self-harm.

Giving them time to work out their true feelings by delaying puberty with hormone blockers saves lives.

Making sure kids have access to treatments that help them be their honest selves SAVES LIVES.

1/ Over the last 21 years as a #trans woman, I've had many unfortunate health care experiences. This is just one. In 2003, I awoke with excruciating back pain and couldn't urinate. After several hours I went to the ER. I was wailing in pain and and was quickly admitted.

It'd been nearly 24 hours since I'd passed any urine and my abdomen looked as if I were carrying triplets. It felt as if I would rupture at any moment. A nurse, recognizing the severity, hurried in with a catheter kit.
3/ She pulled back my hospital gown and saw a penis. She dropped the catheter kit on the floor and walked out. I never saw that nurse again.

The story behind this article is that of a healthy child, born male, but demonstrating the desire to be a girl at age 3.

Her parents placed her in therapy, with the hopes of resolving dysphoria, until age 7. During that time she tried to “behave as a boy”.
At age 9, the child declares herself the female gender and at age 11, she is referred to a “gender identity program”. She has just started male puberty.

I learned a lot. So here's a #medthread on some of the takeaways I found most valuable.

Let's start with our transfeminine pts.

Derm can provide a number of facial feminization procedures (FFP): e.g. botox, soft-tissue augmentation.

These FFPs can be more affordable/less invasive than feminizing surgeries such as forehead or mandibular contouring.

Note: tho traditionally considered "cosmetic", these interventions have MATERIAL psychosocial benefits – studies show that facial feminization has a demonstrated positive impact on QoL + mental health outcomes.

Take the rule "fresh eggs sink in water, old eggs float."

It's a decent rule of thumb. But as a scientist, you examine this to find out why and determine there's some nuance to it...
Fresh chicken eggs generally sink because the shell and its contents are more dense than fresh water. It's a property of buoyancy. Old eggs generally float, because as the egg ages, it chemically reacts to expand and become less dense.

But it's not cut and dry.
Some eggs just happen to be naturally laid with atypical contents and will always sink and always float, regardless of age.

Some eggs are not chicken eggs and those have their own buoyancy.

A lot is going on in the subject of #trans rights, possibly due to "TERF" Jess Philips charges

Trouble is it's all anger, no discussion of the issues, and lots of good old fashioned bigotry and taking of sides.

Despite labour Brexit, I'm going to risk neutrality - 3 points:
1. Should the rights of trans be as important as any other minority group?

2. Are there legitimate concerns for cis and trans in equality of status versus of identity?

3. The "real woman" flashpoint.
If 1 & 2 are resolved is this actually about philosophy not rights?

1. Seems easily solved, the sketchiest review of data says trans are attackes, often more deprived and the victims of sniping by many agendas

I used a standardized array of interview questions across 15 hours of interviews with trans folks regarding their experiences seeking health care in the United States, both POSITIVE AND NEGATIVE, to give providers #narrative examples/guidelines on what not to do, AND what to do.
I did the interviews and my research assistants (@BreanneRBurton, Ren Czachor, Jake Richardson, and Harry Silvey) transcribed them so that no unknown persons would have access to the audio. While some participants gave permission to share audio, all permit sharing transcripts.

The problem w current #trans writers is they avoid talking abt practical matters, like what healthcare model is best for us & what HRT can do—not just fat redistribution but also psychology, behavior & sexuality. Even w/o SRS, I orgasm the female way, capable of multiple orgasms.
Instead, they focus on abstract, theoretical stuff that won't help ppl understand us. I suspect it's bc they're afraid of offending NBs & trans ppl who opt out of HRT. But the fact I mentioned is the best argument that sex *can* indeed change, at least to a considerable extent.
Take @JuliaSerano for an example. She writes in a verbose style & seems to be fond of coining one-ff acronyms that only serve to confuse ppl. If the goal of your writing is to make ppl understand us, you want to do it concise & to the point—not the way current trans writers do.
