"We are concerned that overcrowding and prolonged detention represent an immediate risk to the health and safety of DHS agents and officers, and to those detained."

There have been "security incidents" at "multiple facilities," including escape attempts, the watchdog said.
This is the passage where DHS-OIG quotes a senior manager calling the situation a "ticking time bomb"--the manager and particular facility or facilities are unnamed.
