Aerosol Generating Procedures

Suggested Contents of COVID-19 Intubation Tray

Negative pressure ventilation rooms are ideal to minimize exposure to aerosol and droplets. Where this is not feasible, normal pressure rooms with closed doors are recommended

Pre-Ox: Nasal O2 therapy (via standard or HFNC) should not be used during pre-ox or for ApOx due to the risk of virus aerosolization. If pts are receiving HFNC, it should be turned off prior to removal of patient face mask and nasal cannula to minimize aerosolization

Pre-Ox: If using a BVM, a two-handed vice grip plus a viral filter MUST be inserted between the facemask and manual ventilation devise to minimize aerosolization

Pre-Ox: If using a supraglottic airway, use of a second-generation device is recommended as its higher seal pressure during PPV decreases the risk of aerosolization. Supraglottic airway placement is likely a better option than face mask ventilation due to less aerosolization

Use closed suction systems

Disposable equipment > reusable equipment where available

VL with separate video screen is recommended to maximize distance b/w airway operator & pt’s faces

RSI is default technique: Use >1.5mg/kg IBW of roc or sux for paralytic dosing
