Interesting to see anti #obesity and #diabetes #medication management from the perspective of PharmDs

I'm always pro-collaborative care!

Here are some learning points and fact checks:

#AACE2023 #endocrine #endocrinology #pharmacology

- "Tirzepatide is a dual agonist"
👉It ↑GIP activity but GLP-1R agonism is less than that of endogenous GLP-1
- "Hypoglycemia is not a concern if you're not on insulin"
👉Risk of hypoglycemia ~1.4% vs. 0.2% in placebo in SURMOUNT-1…

- "In this pt with uncontrolled #diabetes, A1c 7.4, we decreased glargine by 10% [when we started tirzepatide]"

👉 Not sure where the evidence is here, but the SURPASS-5 trial protocol reduced basal by 20% in participants with baseline A1c<8…

Vancomycin is primarily effective against Gram positive bacteria ✅

No/clinically insignificant activity against most of the gram negative bacteria.🦠

Because of the large molecular size, vancomycin cannot penetrate the outer membrane of gram negative organisms. ❌

📍In vitro activity against certain nongonococcal Neisseria species 🧫

📍Elizabethkingia meningoseptica that causes meningitis in newborn and immunosupressed. Evidence regarding the use is not clear though. 💉

📌Sedative action is pronounced

#pharmacology #MedStudentTwitter #MedTwitter
📍Cetirizine is a second generation H1 antagonist

📌An active metabolite of hydroxyzine

📌Less sedative action when compared to hydroxyzine but has higher sedative action than most of the other second generation H1 antagonists
📍Levocetirizine is the active enantiomer of cetirizine

📌More potent than cetirizine and can be used at half the dose of cetirizine with lesser sedation

Yesterday, I completed my months-long quest to accurately identify all sci-fields emerging in WW other than the well-established Neuro ⬇️
For, it's been long since we noticed emerging scientific tangents that span diverse fields like: #ML, #Materials_Science, #Pharmacology, #Genomics, #Open_Hardware, etc
Awesome -- But, how does one possibly map all these frontiers without employing actual people to read and annotate dozens of hundreds of abstracts at a time?

I'll be adding short important points which would hopefully help in remembering certain concepts during #PLAB preparation, to this thread🧵every now and then as I take them out from my scattered notes. Feel free to comment your tips and tricks which would help others, as well 👇
Please don't take any of these as medical advice 😂🙏. Real life stuff is much more nuanced. These are just clinchers for the sake of the exam.

What's your favorite anti-seizure medication (ASM)?

If you said levetiracetam (LEV), you're not alone.

Why do so many #neurologists ♥️ LEV? A #tweetorial.

#MedStudentTwitter #NeuroTwitter #MedTwitter @MedTweetorials
What's so good about LEV? First, some #pharmacology:

✅ Fast PO absorption
✅ 1:1 PO:IV 💉
✅ Few med interactions
✅ Can load for status epilepticus Image
And lots of clinical uses:

1⃣ Focal and generalized seizures
2⃣ From 👶 to 🧓
3⃣ Often used in 🤰 (category C):
4⃣ Good for status epilepticus (60 mg/kg, up to 4500 mg) - similar efficacy to phenytoin & valproic acid:

SNEAK PREVIEW: Complete Path Enumeration (CPE) for CRM #dosefinding designs

Featuring a vignette for #Rstats package #precautionary update v0.2-3, currently under review by CRAN. 1/…
This demonstrates the feasibility of a programme I’ve set forth, for the ‘exact’ simulation of ANY dose-escalation design.

Influenced by @MarkusTriska‘s exacting perspective on ’exactness’, however, I now use the more precise term ‘MCSE-free’. 2/
@MarkusTriska ‘MCSE-free’ in turn alludes to the title of recent work by Thomas M. Braun, from which I adapt my application.

(Braun’s mean-field approach truly can be called ‘simulation-free’, whereas I DO simulate the trials—just without Monte Carlo error.) 3/

Had a chance to read in detail this morning, and oh boy do I love this paper! While depression research and MOA of antidepressants are not really my thing now, the whole paradox of how the so-called SSRIs work is a major reason that I got into neuroscience to begin with.
First learned about this in neuropharmacology class as an undergrad @UT_Dallas in 1996 or so from a chapter in Basic Neurochemistry written by Alan Frazer and Julie Hensler. I was captivated by this paradox and decided to apply to @UTHealthSA #Pharmacology for grad school.

For those interested in #cannabis #pharmacology review:… “in this short review we tried to shed light on the potential drug interactions of medicinal cannabis” 1/4 @division_55
“potential outcome of a drug interaction can be additive (1 + 1 = 2), synergistic (1 + 1 > 2), or antagonistic (1 + 1 < 2). Therefore, a drug interaction may lead to an enhanced drug response or modified or unexpected adverse reactions” 2/4
“Cannabis has been used in various forms as crude extracts or purified ingredients (different THC/cannabinoids ratios); therefore, drug interactions caused by cannabis depend not only on drugs involved but also chemical components/profiles of the cannabis preparations used” 3/4

NEW #DTAT paper: What Were They Thinking? Pharmacologic priors implicit in a choice of 3+3 dose-escalation design

ATTN #CBD stakeholders: @US_FDA today holding scientific conf. on CBD/other #cannabinoids. I'll be live tweeting throughout. Not sure we'll get much insight on immediate path forward for consumer products but I'll share noteworthy observations. Agenda:
I'll be paying particular attention to @DrAbernethyFDA's remarks at 9:05a (head of #FDA's #CBD Policy Working Group) and @DThrockmortonMD's keynote at 9:15a (one of Agency's principals for regulating R&D, manufacture & marketing of #cannabis & cannabis-derived #drug products).

My journey to my #PhD program was an absolute #strugglebus. I graduated with a 2.75 GPA from @GeorgiaTech. Most grad applicants have a 3.5 or higher. #Yikes

I applied anyway 🤷🏾‍♀️🤷🏾‍♀️🤷🏾‍♀️ (I didn't get admitted into any programs - post bacc, M.S., or PhD) #AcademicChatter
Fortunately, my mentor @RaquelLieberman gave me the opportunity to work as a #research technician in her lab for 2 yrs to gain the experience I needed for grad school.

This is when I first started studying essential pathways that we could disrupt to kill disease causing bacteria
After working in her lab for almost 2 years..I reapplied again. This time I felt VERY confident I was ready for the #PhD.

I was denied admission to every single grad program I applied to (including the one I'm in now).

What does getting a #PhD actually mean for me?

Here's a video I made last year for a general audience.
"Black Women in #STEM: What I do as a #PhD Student in #Pharmacology"

TLDR: Your ♥️ is regulated by an electrical impulse that travels throughout the ♥️. When this electrical signal is disrupted, this causes your ♥️ to beat irregularly (arrhythmia) & ineffectively pump blood throughout your can imagine that this isn't good 😬.
This irregular heart beat can cause your ♥️ to stop beating entirely = #suddencardiacdeath 🪦! In @EphysWiz's lab, my #PhD focuses on developing a therapy that can correct disrupted electrical activity of the ♥️ to restore healthy heart function. @BlackInCardio #STEMNoire
