They attempted to review the Woods File of 29 FISA Applications. Of those 4 were missing & of those only 1 of the 4 could be proven to have existed. So 3 of 29 may not have even had a Woods File produced.

It has long been a part of FBI culture to let agents bend the rules to help them get the bad guys, because they hunted bad guys who followed no rules.
But it also allows them to go after people they mistakenly believe are bad guys without proper safeguards.

Fortunately, FISC & FBI leadership have already ordered many of these safeguards suggested by OIG to be put in place. This audit shows it was worse than anyone thought.

FBI & FISC are both insisting that mechanisms to continuously measure compliance be implemented also.

A lot of the 'errors' & noncompliance are not new. There are FISC rulings from 2012 that knew the FBI didn't have visibility & compliance procedures to adequately control the process. FISC hoped that the DOJ OIG would be able to improve that process through audits like this one.

But in 2015, Deputy AG Sally Yates prohibited the OIG from auditing the work of the DOJ National Security Division. Effectively removing FISA applications from oversight that the FIS Court felt would improve the process. themarketswork.com/2018/05/28/byp…

Of course it also makes clear that former FBI agents who told us that the FISA process was solid were lying.
