#BuzzWords in #Healthcare that often mean nothing: #PatientEngagement, #PatientCenteredCare #SharedDecisionMaking
Studies have demonstrated for years that IF #patients are #authentically involved in #care outcomes are better, yet, too often the above words thrown out to 1
attract, yet no actions follow.
How often have you been asked your desired outcome of #Healthcare?
How often are you asked about barriers to healthcare or recommendations?
How often are plans focused only on med issue?
How often do you feel your #voice is truly #valued in 2
your own #careplanning?
How often do you feel there is collaboration between all doctors OR do you feel at times doc have competing or conflicting recommendations?

#Patients voice should be central. #LetMeHearYourVoice 3

How do you spell Government Waste?

NEPO!

@kennethpolite of @TheJusticeDept side stepped questions from journalist inquiring as to the costs involved in the new Strike Force.

The @DEAHQ has documented that diversion of RX is rare. Only 1%

justice.gov/opa/pr/justice…
are diverted.

Yes... that's where the Millions of taxpayer's dollars are going!

🧐How coincidental that this strike force targeting physicians & pharmacists was announced the day following the Supreme Court voted 9-0 that the Government must prove beyond a reasonable doubt
that the defendant knowingly or intentionally acted in an unauthorized manner.

The ruling is explained here by @RonChapmanAtty

Would it surprise anyone to learn that @kennethpolite was involved in the #Ruan case?

THIS is #HealthCareInAmerica for patients with #ChronicPain due to @congressdotgov allowing @DEAHQ to regulate the Practice of Medicine through Intimidation, Coercion & unwarranted prosecution of Clinicians & Pharmacists.

Patients making arrangements for #VAD in Switzerland 😭 Image
We can hope that the @USSupremeCourt ruling today on the #Ruan case which required the government to prove beyond a reasonable doubt that the CSA was Knowingly or Intentionally violated will encourage a return to #PatientCenteredCare

However, as long as the @DEAHQ, @CMSGov & Fraud Task Forces continue to target clinicians based on a subjective definition of medical treatment, many may still be hesitant to provide the treatment necessary to restore function & quality of life to patients with #RareDisease,

The options for patients deprived of medical assistance to maintain their health are untenable.

For patients in my daughter's situation where her Seizure RX is involved the outlook is bleak. With less than 10-days remaining to resolve the situation her health & safety are Image
uncertain.

Will she survive? That's our main primary concern.

Will she die of medical neglect or be forced to attempt to self-medicate via the illicit market.

Forced to play a deadly game where all potential *options* may be cost her her life.

There is NEVER a justification for violence towards medical professionals.

Perpetuating Fear & #Stigma is NOT beneficial

It's rare for individuals in #Pain to strike out in violence at medical professionals.

Does it happen? Yes.

Should it? NO!
The ONLY way to prevent future occurrences is to understand WHY it is happening & avoid Fear Mongering & casting aspersions on a population of patients who are experiencing Systemic #Discrimination.

Under & Untreated patients experiencing unremitting pain are more likely to Image
Three cases of people committing such an extreme act of violence is NOT a trend & is NOT reflective of the #PainPatient population as a whole.

There are an estimated 50 million people living with #ChronicPain.

These cases are anomalies, the Root Cause needs to be addressed

The protections for individuals with disabilities under the American's with Disabilities Act are not being enforced when patients with painful diseases report experiencing discrimination.

To influence lawmakers we need everyone to get involved.

Here’s what you can do!

Personally, I find the entire outline of their Orthopedic program concerning from a patient's perspective.

In addition to the "72 hr Pain Free" follow up. It minimizes the potential for patients to experience post-op pain.

Unrealistic expectations shouldn't be advertised🤦‍♀️ Image
Make no mistake, I'm in suggesting this act was justified.

There is no, and can be no justification for striking out at medical professionals when a patient's expectations for treatment are not met.
There has been a lot of discussion regarding the Pain Patient's having unrealistic expectations for Pain Management.

For patients to have reasonable expectations, there must be clear honest discussion of the potential risks / benefits or any procedure including

Now for some Perspective & Rationality over Fear mongering Click Bait headlines

There are Millions Americans living with Chronic Pain who are living with untreated & under treated #Pain since @CDCInjury issued Guidelines for "PCPs" in 2016.

This type of action is an anomaly.
Of the millions of people who have faced systematic discrimination & under treatment, or no treatment, have lost the ability to function & their quality of life there have been 3 instances where they or a family member has committed an act of violence of this magnitude in
retaliation.

More commonly the individual will take their own life rather than lashing out at a medical professional. Image

@pheski @KDrum @PHEski Since my glaucoma diagnosis a year ago this has been a major complaint. (short thread) First: epatientdave.com/2021/12/21/why…
@pheski @kdrum I was aggravated by how hard it is to do this essential thing (instilling eye drops) (why haven't they fixed this???), then further aggravated a few months ago upon discovering this 2017 @Marshall_Allen @Propublica expose of the eye drop business. propublica.org/article/drug-c… 2/
@pheski @kdrum @marshall_allen @propublica The solution to this, and much more, came when last year I found a solid e-patient community, FitEyes. Here's a grab from my first link above: 3/

We've lost too many in this insane '#opioid' war.
The sick & injured are spending the last years of their lives fighting, suffering, begging MDs & GOV for the medicines they need to mediate their pain.
They say taking our medicines away is to 'save' people.
No one is being saved.
Death due to #Untreatedpain or #Undertreatedpain is death by a thousand cuts.

Thread: Thoughts on #valuebased & #patientcenteredcare

Within #healthcare & #advocacy there seems to be a great deal of discussion on both #valuebased care & #PCC. #Healthcare costs have skyrocketed’ & thus a need to #reduce. I am not an #economist but as a #caregiver I 1/
definitely have seen ‘waste’. From my POV costs will continue to skyrocket even if #valuebased care is implemented. One reason (again not an economist) is the vulture ‘istic’ aspect of industries profiting from our system. The other reason is that our system is not centered 2/
around the #patient or patients needs. Thus, if the value based metric is reduced hospital admissions, this, on the face seems straight forth, however if the patients needs are not taken into account but only focused on established metric, costs could (& I will argue often r) 3/

I’m alerting our community to someone too selfish to see the big picture. @thackerpd has attacked @JoshBloomACSH because he actively debunks junk-science aimed at taking away #PatientCenteredCare and redirecting it to the #PainProfuteers
An attack on someone helping painful disabled, acute, and surgical patients is despicable and #Ableist. Paul, I implore you to do more research into things before you go off half-cocked about special interests. How about looking into Chris Christie and Pacira? Or Indivior?
Josh Bloom is smart enough to know EVERYONE will be affected by pain at some point, and no one that has any sense wants to live in a world of unnecessary suffering. Also, his own mother is suffering with pain. Imagine how privileged you must be to not be affected by this.

1/12 Thread: Imagine sitting in a doctor's office/hospital room & being told by a random person there is nothing wrong with you. Now imagine that the person telling you this has an MD after their name. Next, consider that this is not the first time you have heard this message...
2/12 What would you be thinking? What would be going through your mind? You know how you have been feeling. You know your symptoms, your functional limitations created by those symptoms. You know how you felt and how you were functioning prior to a drastic change...
3/12 The test results you have had definitely do not say there is nothing wrong. They say something is off. It could be this or that. It most certainly is not in your head. So why would someone with MD after their name even try peddling such nonsense?

Thank you to @BethDarnall for speaking at #Minnesota @MinnesotaDHS #Opioid Work Group mtg on Thursday: Spoke of importance of #patientcenteredcare, #patientconsent necessary if taper & strongly advised AGAINST force tapers & using pre-determined MME'S. #ChronicPain #NoOneSize
MN #opioid Work Group created Taper Guidance. It will be out 4 public comment soon. Some areas extremely problematic. Although it states NOT 2 taper solely 2 meet system or state policy; MN Quality Improvement requires Drs 2 meet MME thresholds = TAPER TO MEET STATE POLICY. 🤔
If #ChronicPain pt wants 2 try 2 taper from #opioid analgesics 4 any reason & CONSENTS, there needs 2 be safe way 2 do it. Problem throughout U.S, incl #Minnesota; is non-consensual tapering. Many experts, incl #addiction specialists, stress dangers & even state it's UNETHICAL.

1/ It’s been a week.

It’s well into Friday evening & I just got off the phone with a patient who had urgent radiology results. This was after spending > 40 min on the phone w another patient discussing the same this afternoon.

(18 more to come; stick with me.)
2/ I only got 2 items on today’s long “to do” list done — but that’s on me. I was finally able to sit down for a couple hours uninterrupted and dig deep into a few patient cases from the week.

(And I “don’t work” on Fridays.)
3/ As I hung up the phone, I reflected on how #directprimarycare has enabled me to get to the heart of medicine & the doctor-patient relationship.

During that call, my patient’s spouse broke down and — through sobs — simply said: “Thanks for being so kind.”
