I contracted #COVID #Delta variant in early Nov, 2021 which resulted in a #cytokine storm that nearly almost killed me. Here’s how it went. Image
In late Oct, I took the family to Kentucky for a 5-day vaca. Upon returning in early Nov on a Friday, I was feeling fine. Image
By Sunday, I started to get mild symptoms (chills, headache, fever, fatigue). I chalked this up to be a cold / flu which I could sleep off and get better. So I popped a couple of Advils and Tylenols and went to sleep. Temp stayed around 100-101 Image

A few basic facts #COVID19: An infected person, with or without symptoms, can transmit #virus to others. Most may have mild to moderate symptoms. Don't panic. Stay home, isolate, rest, hydrate, paracetamol, monitor temp / oxygen. No steroids, antibiotics. Inform doctor/nurse/ ANM
High fever, incessant cough, breathlessness, oxygen below 93%, other serious symptoms, must see a doctor & follow advice.
Antibiotics, plasma, #Remdisivir medicines with complicated names ending with #MAB have NO effect. Don't take / recommend even if doc insists. Stay Strong.
Persons with serious symptoms requiring hospitalisation, oxygen etc., may have #cytokine reaction & require steroids, anti-coagulants, etc. Unlike other viral pneumonias, #COVIDEmergency is caused by immune system treating virus infected cells as foreign and pours antibodies

offering comprehensive #cytokine profiling (including RANTES levels), to help Drs understand the pathogenesis of Childhood Inflammatory Disease Related to #COVID19 #Leronlimab…
Recent reports in parts of the U.S. and Europe suggest a rare and potentially fatal inflammatory disease linked to #coronavirus is afflicting a small number of children. #Leronlimab may be able to help calm #CytokineStorm
Condition resembles a rare childhood illness called #KawasakiDisease, which has similar signs and symptoms and can lead to enlargement of blood vessels that in severe forms may cause heart damage. #Leronlimab has shown to help calm #CytokineStorm and maybe good #treatment

#Remdesivir begins distribution in the #USA today via #EUA.

I thought would write down some practical #tips based on my #qualitative anecdotal experience after personally treating close to 200 patients with #COVID19.

For quantitative results, wait for the papers¬—thread
“this juice works, doc!” When you hear similar expressions 1-2 days after starting #remdesivir multiple times, you start to wonder. Don't recall a similar proportion of patients smile after feeling so sick 1-2 days earlier with #COVID19. x/n? Wait for the papers.
Like #treatment for any severe infectious disease (meningitis, Gram-negative bacteremia, influenza pneumonia, etc), earlier is always better. #COVID19 is no different. #Remdesivir stops #SARS-CoV2 replication, but won’t heal the lung injury.

#Obesity and #COVIDー19 have been in the headlines recently.

The link between the two is now undeniable.

Let’s take a look at WHY they are so tightly linked molecularly, physiologically, and anatomically in this #tweetorial.

#medthread #medtwitter…
We learned quite a bit about obesity and viral infection in 2009 during the #H1N1 epidemic. Obesity led to:

🔼risk of ICU admission
🔼viral shedding
🔼recovery times

But WHY?…
The story is complex, but inflammation plays a critical role.

Individuals with obesity are in a state of chronic low-grade inflammation as evidenced by altered levels of several cytokines such as TNFα, IL-6, and CRP.

Starting a series on clinical Pearls I am gathering in #COVID19 patients based on experiences of many experts

Will add as we go along

Feel free to add your own observations/experiences

#covidclinicalpearls /1
Anosmia is likely the most specific #COVID19 related symptom
30% of patients have anosmia as their 1st symptom
#covidclinicalpearls /2
Around 90% of patients have fever.
50% maybe afebrile at the time of presentation
Fever tends to be very resistant to routine measures in hospitalized patients
There is no consensus that NSAIDs are to be avoided in #COVID2019 patients
