#Poly_Cystic_Ovary_Syndrome PCOS

PCOS is a HORMONAL DISORDER that affects Women of REPRODUCTIVE AGE (16 - 45 years of age).

1 out of 10 (10%) of ladies are diagnosed with PCOS. 🚺

#PCOS affects woman’s hormonal levels thereby AFFECTING the OVARIES.

A Thread 👇 Image
In HEALTHY WOMEN, the ovaries produce a very small amount of MALE HORMONES called as ANDROGEN.

But In PCOS, due to #INSULIN_RESISTANCE, the ovaries produce EXCESSIVE androgens, RATHER than the female hormones (such as Estrogen & Progesterone). ❌

These Excess #ANDROGENS are
responsible for CAUSING lots of body changes SUCH as : Facial Hair (beards) & No or IRREGULAR menstrual flow.

POLY-CYSTIC means ‘Many Cysts.’

In PCoS, Many CYSTS (Small Fluid-filled Sacs containing IMMATURE EGGS) develops inside the Ovaries.

The thing that makes me the most frustrated about #COVID19 (and has for many months) is the failure to learn lessons from previous outbreaks, particularly about the downstream effects of #pandemic response policy. A thread 1/
This morning @BBCr4today @TheCrick discussed disruption to non-covid #clinicaltrials ; also have seen depressing delays to #cancer detection and treatment ; and changes to routine SRH, maternity, NCD, mental health services.

We need to start talking about the real problem with #AlbertaHealthcare and why @shandro and @jkenney plans will not work, will harm patients, and will cost the system more while delivering less. MDs leaving (and they are) is not the primary problem. 1/x
The biggest issue, is that we started with too few.
I have worked all over this province, and from day one there has been a massive shortage of MDs. Where I work (rural and remote AB) the problem is most apparent. Large % of people unable to get a FM doc. 2/x
Many others driving upwards of 4 hours to see one. Those that are lucky enough to have one? Wait lists are up to 7 MONTHS. The incredible rural docs serving these communities work tirelessly. Routinely > 100 hours a week. But they are human, and can only do so much. 3/x

1/ Q. How does #ContactTracing work again? How is that component of #TestTraceIsolate going so far?

A. Contact tracing is a tried and true #publichealth measure that’s been successfully used to contain other infectious diseases (see graphic). But there are challenges. Image
2/ When adapting this strategy for #COVID19:

First, because people are #contagious even BEFORE developing symptoms, tracing (identifying and notifying contacts) needs to happen quickly. But tracing takes time.
3/ There is also currently no #treatment to offer to individuals who test positive, so there is less personal incentive to cooperate. Instead, contacts are asked to #quarantine, not for their own benefit, but to reduce exposure to others.

Moving average of new cases of #COVID19 and new deaths in #India

#Gujarat has a way higher case fatality ratio than the national value, followed by #WestBengal and #MadhyaPradesh.

We need the CFR to fall to 0!

(1/3)

@aparanjape @c_aashish @MulaMutha @nebuer42 @oommen
In the next 12 states, #Telangana has the worst CFR followed by #Punjab - though the latter is lower than the national CFR.

#Uttarakhand has seen a sudden spike in deaths over the last few days, though luckily the base is low

(2/3)
Deaths due to #COVID19 in #India is yet to really slowdown, we need this to happen.

Early #detection, #quarantine, #treatment & #Masks4All is crucial for this.

Given that we have to live with this for a while, we need to reduce the #deaths to zero!

#Remdesivir begins distribution in the #USA today via #EUA.

I thought would write down some practical #tips based on my #qualitative anecdotal experience after personally treating close to 200 patients with #COVID19.

For quantitative results, wait for the papers¬—thread
“this juice works, doc!” When you hear similar expressions 1-2 days after starting #remdesivir multiple times, you start to wonder. Don't recall a similar proportion of patients smile after feeling so sick 1-2 days earlier with #COVID19. x/n? Wait for the papers.
Like #treatment for any severe infectious disease (meningitis, Gram-negative bacteremia, influenza pneumonia, etc), earlier is always better. #COVID19 is no different. #Remdesivir stops #SARS-CoV2 replication, but won’t heal the lung injury.

Q: What does a #pandemic offensive line look like?

A: The pandemic offensive line has 5 players: #SocialDistancing, #testing, #Isolation, #contact tracing, and #treatment. The goal is to find the virus, trap it, and prevent it from finding other hosts (people) and harming them.
#Social_Distancing makes it harder for the virus to find the next host, but doesn’t tell us where the virus lives. All alone, social distancing is really the defensive line.
#Testing flags the positive cases. With Covid-19, this is very important as many positive cases are asymptomatic allowing this creepy virus to hide and spread.

#Isolation of positive cases, many of whom you would never find without testing, prevents further spread of the virus.

Dermatologist are seeing more and more #skin symptoms related to #COVID19 . Acral lesions (i.e. hands and feet) are particularly frequent, specially in #children and #youngadult with no or few COVID symptoms. After seeing many cases, here is my vision: #Thread 👇
I think that these kind of lesions can be classified in four types. 1. Chilblain-like lesions, which are the most described, usually affecting many fingers / toes, either symmetrically or not. Pruritic and/or painful.
2. Insect-bite like lesions, that may also appear in the palm and the soles. Very #itchy and persistent (more than 2 weeks in some cases)

Starting a series on clinical Pearls I am gathering in #COVID19 patients based on experiences of many experts

Will add as we go along

Feel free to add your own observations/experiences

#covidclinicalpearls /1
Anosmia is likely the most specific #COVID19 related symptom
30% of patients have anosmia as their 1st symptom
#covidclinicalpearls /2
Around 90% of patients have fever.
50% maybe afebrile at the time of presentation
Fever tends to be very resistant to routine measures in hospitalized patients
There is no consensus that NSAIDs are to be avoided in #COVID2019 patients

Yesterday, MDA stations throughout the country held training sessions on how to treat patients, and take samples of those who are suspected of contracting the #Coronavirus.
They practiced taking samples from the patient while wearing full anti-contamination suits, and they have joined the 200 paramedics who have already started to carry out the testing last week.
