I think, if a PM has a vote of no confidence carried against them in the scenario I was describing (Boris Johnson trying to take us out of EU with no deal), he would have to resign straight away
The secenario needs more detail on the earlier stage: let's assume the leaders of opposition parties have agreed with a group of Tory anti-no-deal MPs who the caretaker PM should be
If enough Tory rebels had signed a declaration, it ought to allow the Queen to invite said person to form a government & to put a confidence motion to the House immediately
But this is all another reminder that the Fixed-term Parliaments Act is a truly awful law
I agree with @RobertCraig3's proposal to replace it with a law giving the PM the power to call a general election subject to a simple majority vote of the Commons
