There are four possibilities that should be on the table:
• Mobsters had Epstein killed.
• Corrupt law enforcement had him killed.
• Corrupt politicians had him killed.
• This was simple incompetence.
Mobsters who were involved in Epstein's crimes would have motive (to protect themselves), and easy access to criminal contacts in prison, and possibly to corrupt prison guards.
There's been many rumors of law enforcement help in Epstein coverups, and so they could have motive here too, and would have trivially easy access to corrupt prison guards.
Politicians who were customers (or investers) would also have motive. They would be able to carefully and subtly order a change in Epstein's watch status by trusted allies in the system, and chalk it up to a clerical error in his status.
And finally, people just screw up sometimes.
Which one is most likely? The details of how this happened might point towards which scenario is likely true.

Witness are successfully killed with some frequency even in this country, and few cases see stakes this high. It's foolish to assume that error is most likely cause.
Also note where I said "investors" in parenthesis after customers, there's been some signs that this was Epstein's business model: to coerce customers to invest or recruit, and therefore deepen their implication in the crimes.
