Insulin is NOT a first choice medicine in type 2 diabetes mellitus (T2DM).

Exceptions:
- Short-term use in #T2DM:
(1) ketosis
(2) sepsis
(3) extremely high blood glucose
- Long-term use in #T2DM:
(4) secondary oral medication failure (also called ß-cell failure)

The guideline chart attached above is very clear - lifestyle modifications should be the first choice ± #metformin. Metformin is cheap.
I am least influenced by industry, and use long-term insulin in <5% T2DM, mostly for reason (4), usually after exhausting other options.

2.
Those <5% are at a secondary level care hospital, and not at primary care. In primary care, that number will be even lower.

For those who NEED, both biphasic insulin and glargine are available in @pmbjpbppi #JanAushadhi kendras too. Insulin is quite inexpensive there.

I love to provide structure for my learners. A couple years ago I adopted a set of rules for rounds and ward teams. These continue to evolve.

I share them on day 1, and refer back to them frequently while on service.

Rule 1: We treat patients, not diseases or numbers.

Always always put the disease and number into context for that specific patient. And get to know the patient.

It’s good medicine, and makes the job so much more interesting.

Come prepared and we will get the work done together

Stick with the 4 Ds - decompensated, discharges, diagnostic dilemmas, the C Diff patients last. Stop wherever we are at 11:30 and finish running the list.

The reality is that many, many teachers and schools have fallen victim to the defence/attack mode in the light of new Ofsted perceived expectations, and some of the good work on retention risks being eroded 2/
There are many schools which, far from moving towards a more humane model based on trust are in fact INCREASING performativity measures, and I'm already hearing of new teachers wanting to walk away 3/

As always I start my thread by recognizing my own position in this conversation: I am a (proud) Latina woman training to be a physician-scientist. I hold a lot of privilege having attended an Ivy League institution for undergrad and currently training as a #DoubleDocs at another.
My own alma mater #PrincetonU has been recently grappling with its own (problematic) history by taking active steps to diversify iconography + naming around campus. I am proud to say that this is in part due to a meeting I co-hosted as co-president of PLA in combo with other orgs

DRESS is a life-threatening drug reaction that, as the name implies, can cause systemic/end-organ damage. Although less flashy than SJS/TEN, it's much more common, occurring 10x more frequently! With a mortality estimated to be up to 10%, admission is usually recommended.

Serious question for #medtwitter: If you show up at a code, and the patient is a centaur who had a cardiac arrest, ignoring the joules question, where do you think the defib pads should go? A, assuming the heart is in the human part, or B, assuming the heart is in the horse part?
Do you guys suppose *all* centaurs have the same cardiovascular anatomy? Or are there some that are horse-heart-dependent and some that are human-heart-dependent? I'm guessing human-heart-dependent centaurs would be smaller and more intellectual. Nerd-centaurs, if you will.
