Obvious question: who was at that dinner with Trump who had the authority to fire her, when he said "take her out"? Who would he have been addressing that remark to?

If what he wanted was a straightforward personnel change that was 100% within his power, why'd it take a year?
Let me go a little farther here.

Obviously, yes, it takes Trump a long time to work up the guts to fire someone. But at the point he's telling underlings "I want her gone yesterday", if firing is on his mind, he's there, isn't he?
But he wasn't talking to anyone with the power to fire her. Thank goodness, he also wasn't talking to anyone with the power to kill her. But he was talking to tough guy wannabe mobster cronies, some of whom had been talking up having her surveilled and dangling other options.
Did Trump place a hit on her? No. Careless and shameless though he may be, that's not how he operates. He said something slightly (but not really) ambiguous to someone who might have the ability to act on it, to see what would happen. That's how he operates.
